摘　　要：目的 :研究硝酸甘油 (GTN )舌下片在老年健康受试者和患者体内的药物动力学及药效学。方法 :健康老年志愿者、冠心病患者各 8例 ,含服 GTN舌下片 0 .5 mg,用 GC- ECD法分别于用药前和用药后 2、4、6、7、8、10、15、2 0 m in取血测定 ,以MCPKP药物动力学程序拟合药 -时数据。结果 :本药的处置符合一房室开放模型。两组间进行 t检验 ,在 cmax、AUC方面具有显著性差异 (P<0 .0 5 )。随着血药浓增高 ,平均动脉压 (MAP)、体循环阻力 (TPR)下降 ,心率 (HR)、心脏每搏量 (SV)增高 ,血药浓度与各药效学指标相关性依次为 MAP>HR>SV>TPR。患者与健康受试者之间各药效学指标波动范围 t检验均具有显著性差异 (P<0 .0 5 )。结论 :合理应用 GTN制剂可改善冠心病心绞痛患者的生存质量。AIM: Sixteen subjects (healthy old group 8;patients with coronary heart disease group 8) were given a single dose of 0 5mg glyceryl trinitrate nitroglycerin (GTN) via sublingual way to study pharmacokinetics and pharmacodynamics by gas chromatographic electron capture detection (GC ECD). METHODS: The blood sample were determined before and after 2,4,6,7,8,10,15,20 min treatment, res pectively. MCPKP pharmacokinetic program was applied to imitate concentration time data. RESULTS: The distribution of drug fitted to one compartment open model. There was significant difference in c max and AUC between 2 groups in t test( P <0 05). According to pharmacodynamics, the results showed that following nitroglycerin concentration increasing, MAP and TPR were decreased, HR and SV were increased. The relationship between the blood concentration and pharmacodynamics was MAP>HR>SV>TPR, and there was remarkable difference in t test of wave range of pharmacodynamic indexes between healthy volunteers and patients ( P <0 05). CONCLUSION:In this study we make Chinese nitroglycerin pharmacokinetic model to instruct the clinical doctors to use nitroglycerin betterly to improve patients' life quality to decrease sudden death.
