摘 要:目的 探讨血管内皮生长因子(VEGF)和血管生成素-1(Ang-1)基因联合转染对兔心肌梗死部位再生血管分支数量和毛细血管管径大小的影响.方法 将75只日本大耳白兔随机分为5组(每组n=15),采用冠状动脉左前降支结扎法建立兔心肌梗死模型.单纯梗死组,仅行冠状动脉左前降支结扎术;VEGF组,向心肌梗死边缘部位注射2型重组腺相关病毒携带的VEGF165基因; Ang-1组,向心肌梗死边缘部位注射2型重组腺相关病毒携带的Ang-1基因;VEGF+Ang-1组,向心肌梗死边缘部位注射2型重组腺相关病毒携带的VEGF165基因和2型重组腺相关病毒携带的Ang-1基因混合液(混合比例为1:1);载体组,向心肌梗死边缘部位注射不携带VEGF165和Ang-1基因的2型重组腺相关病毒载体溶液.8周后取材,植物凝集素B4(Lectin B4)灌注染色检测心肌梗死边缘再生血管分支数量,CD31免疫组化检测毛细血管直径.结果 与单纯梗死组、Ang-1组和载体组相比,VEGF组和VEGF+Ang-1组再生血管分支增多(P〈0.01);与其他各组相比,Ang-1组再生血管分支显著减少(P〈0.01).与单纯梗死组、VEGF组和载体组相比,Ang-1组和VEGF+Ang-1组梗死边缘区毛细血管直径显著增加(P〈0.01);与其他各组相比,VEGF组梗死边缘区毛细血管直径减小(P〈0.01).结论 VEGF和Ang-1联合转染可以促进心肌梗死边缘部位再生血管分支增多和毛细血管管径增大.Objective To investigate the effect of combined transfer of vascular endothelial growth factor (VEGF) and angiopoietin - 1 ( Ang - 1 ) genes on the number of angiogenic sprouting branches and capillary diameter after acute myocardial infraction in rabbits. Methods Acute myocardial infarction model was established by left anterior descending artery ligation and then the 75 Japanese white rabbits were randomly divided into 5 groups ( n = 15 each). Pure infarction group: nothing was injected. VEGF group: VEGF165 plasmid DNA was injected around the periphery of the myocardial infarction after the ligation. Ang - 1 group : Ang - 1 plasmid DNA was injected around the periphery of the myocardial in- farction. VEGF + Ang- 1 group: VEGF165 and Ang- 1 plasmid was injected. Vector group: plasmid without VEGF165 or Ang- 1 gene was injected. Hearts were removed and prepared into sections 8 weeks after operation. Expression of VEGF and Ang - 1 gene was detected by Western blot. Lectin B4 staining and CD31 immunohistochemistry were used to evaluate angiogenic sprouting branches and capillary diameter respectively. Results Compared with pure infarction group, Ang - 1 group, and vector group, the number of angiogenic sprouting branches increased siginificantly in VEGF group and VEGF + Aug - 1 group (P 〈 0.01 ). Compared with other groups, the number of angiogenic branches de- creased significantly in Ang - 1 group ( P 〈 0.01 ). The capillary diameter at the periphery of myocardial infarction in- creased siginificantly in Ang - 1 group and VEGF + Ang - 1 group compared with pure infarction group, VEGF group, and vector group (P 〈 0.01 ). Compared with other groups, the capillary diameter at the periphery of myocardial infarction decreased siginificantly in VEGF group (P 〈 0.01 ). Conclusion The combined gene transfer of VEGF and Ang - 1 can effectively improve the perfusion of the myocardial infarction area by increasing the angiogenic sprouting branches and capillary diameter at
