特发性血小板减少性紫癜中医药治疗探讨

摘　　要：特发性血小板减少性紫癜(ITP)系血小板免疫性破坏,外周血中血小板减少的出血性疾病。该病反复发作,不易治愈。笔者通过文献研究并结合临床经验从病因病机、辨证分型与治法、专方专药治疗等方面,系统论述了中医对该病的认识,治疗优势及自己的见解;并阐述了难治性ITP的主要病机为脾肾两虚,气血亏虚,兼有瘀血,病位主要在脾、肾;提出治以补肾健脾,调理气血,祛瘀生血为法的新思路,创建了芪龙调血方,对该病的中医药治疗具有重要指导意义。Idipathic thrombocytopenic purpura（ITP） is a hemorrhagic disease with immunity destruction of platelets and platelets decrease in the peripheral blood. The disease is hard to be cured for repeated outbreak. Through studying the documents and combining the clinical experiences in the aspects of etiology, pathogenesis, syndrome differentiation, therapy and special formula, this article expounds the understanding of ITP and superiority in TCM therapy of ITP, then elaborates that the main pathogenesis of ITP is deficiency of kidney and spleen, as well as depletion of qi and blood with the blood stasis, the location of ITP is based on spleen and kidney. And this article also states the new thought to cure the recurred ITP in the way of replenishing kidney and strengthening spleen, regulating Qi and blood, removing the blood stasis. Based on the new thought, the author creates Qilong Tiaoxue Decoction which plays an important role in the treatment of ITP in TCM.
