摘 要:目的 :以超声造影剂 FX5 30作为指示剂 ,以视频强度的变化作为指示指标 ,经外周静脉团注 FX5 30 ,根据相关数学模型确定定量测定肾皮质血流灌注量的方法。方法 :1)建立肾动脉狭窄模型 ,对不同程度的狭窄进行超声造影实验 ,并以组织血流仪测定肾皮质部位在各个程度的实际血流量 ;2 )根据指示剂稀释原理测定组织的单位容积内所含血管管腔内的血流容积比例与造影剂通过该单位容积组织的平均渡越时间 ,计算组织血流灌注量 ;3)比较组织血流仪测定值与本方法测定值之间的差异性和相关性。结果 :五只家兔试验 ,本研究测定值各级别为 :0级 :36 7.2± 5 3.9; 级为 :338.8± 42 .4; 级为 :184.6± 137.8; 级为 :0。组织血流仪测定值分别为 :0级 :314.3± 35 .6 ; 级 :190 .2± 73.5 ; 级 :6 5 .1± 47.4; 级 :0。两种方法的相关性研究表明 ,我们的方法测定值与组织血流仪测定值相关良好 (r=0 .74,P<0 .0 5 )。结论 :指示剂稀释法和声学造影剂结合用于定量检测肾皮质血流灌注量是一准确而可靠的方法 ,具有广泛的应用价值。Objective:This study is try to find a new method to calculate the kidney perfusion based on Power Doppler Image(PDI)by bolus intravenous contrast FX530 according to the indicator dilution theory.Methods:1)The dynamic images of kidney were captured by video capture card in PC.2)The time intensity curve of every stenosis level in the different position were analyzed by using our designed software.In every level,we use our method to calculate the flow comparing with the tissue flow obtained from measurer equipment.3)The kidney cortical perfusion flow is evaluated using the indicator dilution relation,accoring to which flow equals the ratio between the intravascular fraction of the tissue sample volume and the mean transit time of the contrast agent.Results:By our method,the every level perfusion is following:level 0=367 2±53 9;level Ⅰ=338 8±42 4;level Ⅱ=184 6±137 8;level Ⅲ=0. By the tissue flow measurer equipment:level 0=314 3±35 6;level Ⅰ=190 2±73 5;level Ⅱ=65 1±47 4;level Ⅲ=0.By calculating the relationship between the two methods,our method correlates well with the standard method( r =0 74, P <0 05).The parameters among the two methods are not significantly differnet.Conclusion:Combination of ultrasound contrast and indicator dilution method is valuable for examining kidney perfusion.
