摘 要:目的 :探讨细胞凋亡在妊娠维持中的作用。方法 :采用凝胶电泳技术及 S- P免疫组化技术 ,对 2 0例早期妊娠避孕失败者 (A组 )及 10例自然流产者 (B组 )的绒毛及蜕膜组织 ,进行 DNA片段分析、p5 3基因产物 (Tp5 3)、Fas-配体 (Fas- L )蛋白基因产物表达检测。结果 :A组 2 0例有 80 %蜕膜组织出现 DNA梯形条带 ,而绒毛组织则无 ;B组全部绒毛及蜕膜均出现 DNA梯形条带 ;Tp5 3在 A组绒毛及蜕膜组织中均未见表达 ;在 B组中表达阳性率为80 % ,二组比较有明显差异 ;Fas- L 在 A组绒毛及蜕膜组织中的表达阳性率分别为 40 %、5 0 % ;在 B组绒毛及蜕膜组织中均为 10 0 % ,两组绒毛间、蜕膜间比较均有明显差异 ;Fas- L 在绒毛中阳性表达的细胞为滋养细胞 ,在蜕膜中阳性表达的细胞为蜕膜细胞及腺管皮细胞。结论 :早期妊娠中 ,蜕膜细胞以凋亡形式死亡 ,在胚泡植入及胎盘形成过程中发挥重要作用 ;在自然流产中 ,绒毛及蜕膜细胞的凋亡受 p5 3基因及 Fas- L的共同调控 ,凋亡过度可能是其发病的分子机制。Objective:To study the mechanism of decidual regression and the gene control of decidual and placental villi apoptosis.Methods: 20 deciduas and chorionic villi in early gestational stage (6 8 weeks)were obtained from artificial abortion (group A) and 10 spontane ous abortion (group B) respectively. DNA fragments was analysed by gel electrophoresis; The p53 protein and Fas Ligand expression in the samples were investigated immunohistochemically by S P methoed.Results: The nucleosomal DNA fragments was observed in 80% of decidual tissue in group A; But none of chorionic villi shows the DNA ladder which was detected both in the decidual and chorionic villi in group B. No p53 expression was detected during decidual apoptosis in group A; the positive rate of p53 in group B was 80% (P<0.005). Fas ligand expression rate in decidual and chorionic villi was 40% and 50% respectively in group A. In group B, the positive rate of Fas ligand expression was 100% both in decidual and chorionic villi (P<0.025; P<0.005).Conclusion: These results provide biochemical evidence that decidual regression occurs by apoptosis, Fas ligand has been associated with decidual apoptosis. Defective regulation of decidual and chorionic villi apoptosis may play a part in the aetiology of spontanous abortion, which involves the common regulation of p53 and Fas ligand.
