摘 要:目的探讨血管回声跟踪(ET)技术检测正常高值血压者颈总动脉弹性功能的变化情况及其临床意义。方法应用ET技术检测63例正常高值血压者(HNBP组)与50例健康成人(正常对照组)双侧颈总动脉的弹性参数,包括弹性系数(β)、僵硬度(Eρ)、顺应性(AC)、增大指数(AI)及单点脉搏波传导速度(PWVβ),并使用常规血管超声测量颈动脉内-中膜厚度(IMT),结果进行对比分析。结果组内双侧颈总动脉的IMT及其各弹性参数比较差异均无统计学意义;两组间上述各指标比较,HNBP组的β、Eρ和PWVβ增高,AC降低,差异均有统计学意义(P<0.01),AI和IMT无明显变化,差异无统计学意义。结论正常高值血压者颈总动脉Eρ增加,ET技术能较早发现颈总动脉弹性的改变,对临床早期预防心脑血管疾病的发生具有重要意义。Objective To investigate the changes of carotid arterial elasticity in patients with high-normal blood pressure by echo tracking technique (ET). Methods Sixty-three patients with high-normal blood pressure and 50 health subjects were enrolled in this study.Their carotid elastic parameters were detected by ET, including stiffness parameter (β), pressure-strain elastic modulus ( Eρ ), arterial compliance ( AC ), augment index ( AI )and pulse wave conducting velocity ( PWVβ ). The intima-media thickniss (IMT) was measured by routine vascular ultrasound. The results were compared and analyzed. Results There was no difference in carotid elasticity and the blood pressure between left and right side. Compared with control group, β, Ep, PWVβ were increased conspicuously in high-normal blood pressure group, AC was lower significantly (P 〈 0.05), there was no significant difference of AI and IMT between the two groups. Conclusion Ep of the carotid artery in patients with high-normal blood pressure is increased, while βis decreased. ET is more superior than routine vascular ultrasonography, and may reflect the changes of carotid artery early, it has significance to prevent cardiac and cerebrovascular disease.
