三种综合疗法对瘢痕疙瘩疗效的对比分析

摘　　要：目的:探讨比较三种综合疗法对瘢痕疙瘩的治疗效果、不良反应和患者满意度的差异。方法:对2006年5月～2010年12月期间收治的121例瘢痕疙瘩病例进行回顾性分析,将满足纳入标准的89名病例按照治疗方法分成A组(接受手术联合放射治疗组)、B组(接受手术联合皮质类固醇激素治疗组)和C组(接受皮质类固醇激素联合放射治疗组),对比分析各组在疗效、不良反应与患者满意度的差异,以P>0.05为具有统计学意义。结果:三组的总有效率分别为:83.87%、81.25%和61.54%,A组疗效优于C组(χ2=8.035,P=0.018),B组疗效优于C组(χ2=8.290,P=0.016),但A、B两组的疗效之间不存在差异(χ2=0.162,P=0.992)。三个组的不良反应均较轻微,均由单独治疗方法引起的,未发现各疗法之间存在交互不良反应。三组患者满意度均不低于75%,且各组之间的满意度不存在差异(χ2=7.416,P=0.115)。结论:三种综合疗法在治疗瘢痕疙瘩方面均有较好疗效,其中手术联合放射治疗法和手术联合皮质类固醇激素治疗法的总有效率较高。Objective To explore the differences of three synthetical treatments of keloid by comparing the treatment effect, side effect and the degree of patients＂ satisfaction.. Methods 121 cases in our hospital from May 2006 to December 2010 had Seen under retrospective analysis, and 89 cases of them were selected as the study objects according to inclusive criteria By the criteria of different therapeutic methods these selected cases were divided into three groups --Group A, Group B and Group C, and the synthetica^l treatments are respectively operation combined with radiotherapy, operation combined with cortisol treatment and cortisol treatment combined with radiotherapy, for the purpose of comparative analysis of the treatment effect, side effect and the degree of patients＇ satisfaction of three synthetical treatments of keloid. Results The rate of treatment effect of the groups were respectively 83.87%,81.25% and 61.54%. rate of treatment effect in group A was significantly higher than that in group C, rate of treatment effect in group B was also significantly higher than that in group C, but no significant difference was found between group A and group B. Side effects in all the groups were slight, and caused by the single treatment, not by therapies＂ interaction. No degree of satisfaction was under 75%, and no significant difference was found between any tow groups. Conclusions These applied synthetical treatments of keloid are all with good effects, surgery combined with radiotherapy and surgery combined with corticosteroids have higher total effective rates.
