老年消化性溃疡患者Ghrelin和Leptin血清水平及组织表达的意义

摘　　要：目的探讨老年消化性溃疡患者生长激素释放多肽（Ghrelin）和瘦素（Leptin）血清水平及组织表达的意义。方法选取老年消化性溃疡患者59例，其中老年十二指肠溃疡组（DU）30例、老年胃溃疡组（GU）29例，另择正常对照组30例，采用ELISA法检测血清Ghrelin和Leptin水平，同时用免疫组化法检测胃黏膜中Ghrelin及Leptin的表达。结果老年DU及GU组血清Ghrelin水平高于对照组，差异均有统计学意义（均P〈0．05），GU组血清Leptin水平低于对照组，差异有统计学意义（P〈0．05）．老年DU组胃黏膜Ghrelin表达阳性率（83．3％）及老年GU组胃黏膜Ghrelin表达阳性率（86．2％）均明显高于正常对照组（60％），差异均有统计学意义（均P〈0．05）。结论Ghrelin与老年消化性溃疡的发病有关，血清Leptin水平降低可能与老年GU的发生有关。Objective TO investigate of expression of ghrelin and leptin in serum and gastric mucosa in elderly patients with peptic ulcer. Methods Eighty nine elderly patients with peptic ulcer, including 30 cases of duodenum ulcer（DU） and 29 cases of gastric ulcer（GU） were enrolled in the study; 30 healthy subjects served as controls. Serum levels of ghrelin and leptin were measured by ELISA method, and expression of ghrelin and leptin in gastric mucosa was detected by immunohistochemistry. Results Serum levels of ghrelin in groups GU and DU were significantly higher than those in control group（P〈0.05）; while serum level of leptin in group GU was significant lower than that in control group（P〈0.05）. The positive expression rates of ghrelin in group DU（83.3%） and in group GU（86.2%） were significant higher than that in control group（60%）（P〈0.05）. Conclusion Ghrelin might be related to pathogenesis of peptic ulcer in elderly patients, and lower serum leptin might be associate in occurrence of gastric ulcer in elderly patients.
