摘  要:目的探讨慢性乙型肝炎患者HBVP区常见耐药位点Rt204±180以外位点变异与拉米夫定耐药关系。方法收集27例接受拉米夫定初治患者治疗基线和获得病毒学应答后出现病毒学反弹的血清标本,采用PCR产物直接测序技术,检测治疗基线和病毒学反弹时HBV DNA的P区变异位点,对位点变异与拉米夫定耐药关系进行分析。结果 HBVP区的rtA181V、rtM171R、rtN/H238T/A和rtI187V变异不伴随有P区rtM204V/I±rtL180M耐药位点的变异。结论 HBVP区的rtA181V、rtM171R、rtN/H238T/A和rtI187V变异可能是拉米夫定耐药的新变异位点。Objective To investigate the relationship between other sites mutation except HBV PRt204+180 and Lamivudine-resistance.Methods Fifty-four serum samples of 27 patients with chronic hepatitis B at baseline and HBV DNA rebound after Lamivudine therapy were collected,mutations at baseline and HBV DNA rebound in polymerase(P) mutation site and the relationship between the site mutation and Lamivudine-resistance were analyzed by polymerase chain reaction(PCR) direct sequencing.Results The following mutations were not accompanied by the rtM204V/I±rtL180M resistance wite mutations.They were gene mutations(rtA181V,rtM171R,rtN/H238T/A and rtI187V).Conclusion The following mutations in HBV P,including rtA181V,rtM171R,rtN/H238T/A and rtI187V,may be the new mutation sites related to Lamivudine-resistant and related to Lamivudine-resistance.
