先天性髋关节脱位的一期手术治疗

摘　　要：目的　改进先天性髋关节脱位的治疗。方法　对 38例 5 2髋先天性髋关节脱位术前不牵引 ,一期综合手术矫正全部畸形。手术步骤包括彻底软组织松解 ,股骨粗隆下短缩、去旋转、内翻截骨 ,髋臼覆盖重建 ,准确地同心圆中心关节复位。结果　本组 5 2髋术后均获得满意复位 ,其中 30例 41髋随访 8～ 30个月 ,按Muller和Seddon的标准 ,优2 8髋 ,良 10髋 ,可 3髋。结论　一期综合手术治疗效果满意 ,具有住院时间短、病人痛苦小等优点 ,值得推广。Objective: To improve the treatment for congenital dislocation of the hip. Methods: Without preoperative traction, one stage combined operations were performed in 38 patients, to correct all the deformities of 52 hips of congenital dislocation of the hip simultaneously. The steps of the procedure included complete soft tissue release, shortening, derotation and varus subtroc hanteric osteotomy, reconstruction of acetabular roof and concentric reduction of the hip. Results: All the 52 hips got satisfactory reduction after the operations. Of the 38 patients, 41 hips in 30 cases were followed up from 8 to 30 months. According to the assessment of Muller and Seddon, excellent results were got in 28 hips, good in 10 hips, fair in 3 hips. Conclusion: It is suggested that the one stage combined operations can have satisfactory outcomes and the advantages of shortening hospitalization and diminishing patients' suffering,and should be spread in clinical practice.
