联合髋关节成型术治疗先天性髋关节脱位36例

摘　　要：目的探讨解决先天性髋关节脱位一次性手术复位的方法。方法根据本组 36例先天性髋关节脱位(congenitaldislocationofhip ,CDH)临床表现和病例特点 ,选择联合髋关节成型术 ,采用切开复位 ,股骨近端发育不良矫正 ,髋臼周围截骨术。结果随访 1～ 9年 ,36例中疗效优者 13例 ,良者 18例 ,差者 3例 ,失败者 2例。结论通过临床及X线表现观察发现CDH诸多病理改变中髋臼指数增大 ,股骨近端发育不良是多数CDH共同特点。采用联合髋臼成型术一次性解决CDH存在的病理改变 ,减小髋臼指数 ,增加包容 。ObjectiveTo investigate the reducing method for treating congenital dislocation of hip(congenital dislocation of hip,CDH).Methods36 cases were performed of united arthroplasty of hip to open reduction, modify proximal femoral dysplasia and acetabuloplasty.Results36 cases were followed up over 1-9 years. Excellent effect obtained was in 13 cases, good in 18 cases, poor in 3 cases, failure in 2 cases.ConclusionThe x ray and clinical findings demonstrate that proximal femoral and acetabular dysplasia should be the main pathological changes of the dislocation. The combined arthroplasty is beneficial for a stable reduction of CDH.
