摘 要:目的 探讨乌斯他丁(UTI)改善高压电烧伤病人皮瓣微循环的疗效.方法 明确诊断的高压电烧伤病人40例,随机分为常规治疗组(对照组)和UTI治疗组(治疗组),每组20例.对照组按国内烧伤常规补液复苏方法治疗.治疗组按国内烧伤常规补液复苏方法结合UTI(每次120万U,静点,每日2次)治疗.用激光多普勒微循环图像仪检测2组病人入院时治疗前及治疗后48h创面微循环灌流量(MPF),观察上述时相创面毛细血管充盈时间及创面愈合时间.对2组治疗前后的观测结果进行对比分析,判断治疗效果.结果 对照组和治疗组治疗48h后创面MPF分别是(2.04±0.56)V、(2.56±0.73)V,均大于治疗前的(1.62±0.70)V、(1.54±0.48)V;2组临床表现为创面血运好转.治疗组治疗48h创面血运优于对照组,创面愈合较对照组缩短.结论 UTI对高压电烧伤病人创面微循环障碍有改善作用,缩短电烧伤创面愈合时间.Objective To explore the therapeutic effects of Ulinastatin(UTI) to wound microcirculation in High-Voltage Electrical Burns(HEB) patients.Methods Forty HEB patients(men and women were equal) were randomly divided into control group and treatment group,each group twenty peoples,average age (32.02 ± 15.34) years old.The control group were treated with universal fluid replacement.The treatment group were treated with UTI(1.2 million U,intravenous drip,two times per day),in addition to universal fluid replacement.The LISCA Laser Doppler Hemoperfusion Image instrument(LDPI) was used to observe the changes of wound microcirculation perfusion(MPF) at admission instantly and 48 hours after treatment,and to detected capillary filling time and healing time of wound, and then to analysis the observation results and judge therapeutic effect. Results The MPF values of control group and treatment group were respectively(2.04 ± 0.56)V,(2.56 ± 0.73)V at 48 hours after treatment,which were respectively more than (1.62±0.70)V,(1.54 ± 0.48)V at admission instantly. The wound blood supply of both groups improved,the wound blood supply of treatment group was better than control group,the wound healing time was shorter than control group.Conclusion UTI can improve wound microcirculation of HEB patients, and shorten wound healing time.
