异体真皮脱细胞基质在男性尿道重建中的临床应用

摘　　要：目的评价异体真皮脱细胞基质在男性尿道重建中的临床应用。方法回顾分析12例采用异体真皮脱细胞基质重建尿道患者的临床资料。结果该组12例尿道狭窄患者分别于术后4周后拔除尿管,自行排尿顺利,最大尿流率13～18 mL/s。定期扩张尿道。3例因局部感染致瘢痕形成再次给予等离子柱状电极内切开,效果满意。余9例患者随访1年,均排尿通畅,未再发生尿道狭窄。结论异体真皮脱细胞基质组织相容性好,手术操作简便,在尿道重建中安全有效。【Objective】 To investigate the value of acellular dermal matrix（ADM） for urethral reconstruction in man.【Methods】 12 cases underwent acellular dermal matrix for urethral reconstruction in man were analyzed retrospectively.【Results】 All 12 patients had satisfactory voiding after removing the catheter after 4 weeks,and maximum urinary flow rate（Qmax） were 13~18 mL per second..Except 3 cases developed stenosis postoperatively were operated by internal urethrotomy with bipolar plsamakinetic.9 patients were followed up for 12 months,all patients voided normally.【Conclusion】 Homologous acellular dermal matrix has better histocompatibility.Homologous acellular dermal matrix in the treatment of urethral reconstruction in man is safe and effective.
