超声引导徒手穿刺在经皮肾镜取石术中的应用

摘　　要：目的探讨超声引导徒手’肾穿刺在经皮肾镜取石术（PCNL）中的临床应用。方法使用B型超声仪引导，行徒手肾穿刺造瘘术治疗57例肾多发结石患者。结果47例患者均一次穿刺造瘘成功，45例患者一次取尽结石，1例因肾周大量积液中止手术，术中未出现大出血等患者。结论超声引导徒手肾穿刺准确、安全，造瘘瘘道宜于肾镜操作，易于取石。Objective To evaluate the clinical application of percutaneous endoscopic nephro- lithothomy （PCNL） under ultrasound guidance. Methods The ultrasound-guided PCNL was performed in 57 patients with renal caculi were studied. Results Except 1 patient with massive circumrenal fluidi- fy, under the guidance of ultrasonography the puncture was completed at one session in all 57 patients. In the cases, 45 patients were cured after the first operation, no massive hemorrhage or other severe compli- cations occurred. Conclusions The unarmed ultrasound guidance is an accurate and safe method which is valuable in endoscopic nephrolithothomy application.
