无水乙醇换洗联合医用生物蛋白胶治疗巨大肾囊肿临床观察

摘　　要：目的探讨无水乙醇换洗联合医用蛋白胶注射治疗巨大肾囊肿的临床效果。方法巨大肾囊肿39例均行B超引导下经皮、经肾囊肿穿刺置管术。根据囊肿直径分为3组：A组26例（10～15cm），B组9例（15～20em），C组4例（〉20cm）。每次注入无水乙醇1～2次，注入量为囊肿体积的1／4—1／3，当囊肿引流量〈10ml／d并经B超检查囊肿直径3～6cm，注入医用生物蛋白胶5—10ml后拔除置管。结果所有患者均一次性穿刺成功，成功率100％，术后均放置导管引流。A组囊肿全部闭合时间为30d，B组为70d，C组随访12个月仍未闭合，但均明显缩小，直径〈6cm。本组治愈率89．7％，有效率100％。结论B超引导下经皮、经肾囊肿穿刺置管无水乙醇换洗联合医用蛋白胶注射治疗巨大肾囊肿方法简单、微创、完全性高、疗效确切。Objectives To investigate the effect and safety of the treatment of giant renal cyst with color doppler ultrasound guided pereutaneous puncture catheterization drainage and absolute ethanol combined with injec- ting Biological Fibrin Glue. Methods 39 cases with giant renal cyst were treated with color doppler ultrasound guided percutaneous puncture catheterization drainage and absolute ethanol combined with injecting Biological Fibrin Glue treatment. Patients were divided into 3 groups according to cyst diameter. In groups A（26 cases） , cyst diameters were from l0 cm to 15cm, in group B（9 cases） ,from 15 to 20 cm, and in group C （4 cases） ,more than 20cm. Results Puncture was finished and the success rate was 100%. All patients were put central venous catheter. In groups A, cysts totally disappeared about 30 days later. In group B, around 70 days later and in group C, follow up visits showed that the cysts didnt disappear until 12 months later. Howeve, these cyst~ diameter were smaller than 6cm. The cure rate was 89.7% and the effective rate was 100%. Conclusions The treatment of gaint renal cyst with color doppler ultrasound guided percutaneous puncture catheterization drainage and ethanol combined with injecting Biological Fibrin Glue was an effective and safe method for the patients with renal cysts. We recom- mend it as the first therapeutic option in these patients.
