摘 要:目的探讨血清、尿半胱氨酸蛋白酶抑制剂C(Cystatin C)在糖尿病肾病(DN)早期诊断中的价值。方法测定糖尿病正常蛋白尿组(A组),糖尿病微量蛋白尿组(B组),糖尿病大量蛋白尿组(C组)的血清、尿Cystatin C,尿α1微球蛋白(α1-MG),血清肌酐(SCR)和血尿素氮(BUN),比较各项指标。结果血Cystatin C值C组明显高于A、B组,B组高于A组,Cystatin C在3组之间差异均有统计学意义(P<0.05);BUN、SCR在A、B组中无显著性差异。各组间尿Cystatin C值的差异均有显著性(P<0.05)。患者尿的Cystatin C和α1微球蛋白的水平呈正相关(γ=0.81,P<0.01)。结论血清、尿Cystatin C分别是判断早期DN患者肾小球功能和肾小管受损的敏感指标。Objective To explore the value of serum and urine Cystatin C in the early stage of type 2 diabetic nephropathy.Methods 74 type 2 diabetic patients were divided into normal albumiuria group(A),microalbumiuria group(B) and macroalbumiuria group(C) according to urine albumin excretion rate(UAE).Serum and urine Cystatin C,α1-microglobulin(α1-MG)was determined by particle enhanced immunoturbidimetry.Serum creatinine(Scr),and blood urea nitrogen(BUN) were detected simultaneously.Results Serum Cystatin C level of the group C was significantly higher than group B and A,the value was also different betweet group A and group B(P0.05).but the value of BUN,Scr in the group A and B did not show differences.The value of urine Cystatin C and α1-MG show a high Correlation(γ=0.81,P0.01).Conclusion Serum and urine Cystatin C is a sensitive marker to estimates glomerular filtration and tubular dysfunction of early diabetic nephropathy respectively.
