乌鲁木齐儿童乙肝疫苗接种后无(弱)应答影响因素的病例对照研究

摘　　要：目的研究乌鲁木齐5岁以下儿童在接种乙肝疫苗(Hepatitis B Vaccine,HepB)后出现无(弱)应答的影响因素。方法按性别、年龄、民族1∶1配对原则,选取279对无(弱)应答者病例和对照,采用多因素条件Logistic回归分析父母健康状况,疫苗接种剂量、种类、部位、次数以及药物摄入、饮食等因素与无(弱)应答发生的关系。结果多因素条件Logistic回归分析显示与无(弱)应答有关的因素为母亲HBsAg阳性、母亲HBsAb阳性、父亲HBsAg阳性、父亲HBsAb阳性、父亲有HepB免疫无应答史,其OR值分别为4.467、0.879、5.768、0.881、6.020,其中父亲HBsAb阳性、婴儿餐具进行消毒,出生当日接种疫苗是儿童体内产生HBsAb的保护因素。结论父母亲为HBV携带者是儿童在接种乙肝疫苗后发生无(弱)应答的危险因素,而父母亲体内有HBsAb是儿童在接种乙肝疫苗后产生应答的保护因素。Objective To investigate the factors of non-or low-response to hepatitis B vaccine among chil dren under 5 years old in Urumqi. Methods The case-control study was conducted on both 279 cases and controls under the same distribution of sex, age, ethnic group, residence. Conditional multivariate Logistic regression was used to explore the relationship between no/low response of HBsAb and study factors a bout parents health, dosage and type of vaccines, injection position and times, drug use, diet and so on. Results Conditional multivariate Logistic regression analysis showed the effective factors include both HBsAg and HBsAb positive in serum of mother and father, father had a history of no response after Hepa titis B vaccination, and the values of OR were 4. 467, 0. 879, 5. 768, 6. 020 separately. HBV carrier in fa ther, infant waredisinfection, HBV inoculation was born on that day is the protection of children produce second liver antibody factors. Conclusion That the parents who are HBV carriers is a risk factor to influ ence the response of Hepatitis B vaccine in children′s serum. Whereas if the HBsAb is positive in parentsr serum, the reason will be as a protective factor to conduct the response after Hepatitis B vaccine injection for children.
