温水泡手配合活络油外搽按摩运动预防化疗性静脉炎的效果观察

摘　　要：目的探讨采用温水泡手配合活络油外搽按摩运动预防化疗性静脉炎的方法及效果。方法以80例妇科化疗患者为观察对象,随机分为实验组和对照组各40例,对照组于每次化疗前常规滴注葡萄糖溶液500～1 000 ml进行水化,化疗后用生理盐水100 ml冲管。实验组:在常规护理的基础上,于每天化疗前用温水泡手及化疗后用活络油外搽按摩上肢并进行上肢上举、扩胸运动,观察记录两组患者化疗5个疗程内发生化疗性静脉炎的例数及疗效,以及静脉穿剌次成功率。结果实验组患者在化疗的5个疗程内发生化疗性静脉炎例数少,在静脉显露易见、弹性好、压痛轻、滴注通畅方面明显优于对照组,静脉穿剌成功率高,与对照组比较,p<0.01,有统计学意义。结论化疗前用温水泡手,化疗后用活络油外搽上肢血管行走部位,配合按摩上肢及增加上肢运动,可预防化疗性静脉炎,提高静脉穿剌成功率,减轻患者痛苦。Objective To explore the effect of hand dunking in warm water and Huolo oil external applica- tion associated with massage and exercise on preventing chemotherapy phlebitis. Methods 80 cases of gynecological patients treated with chemotherapy for the observed object were randomly divided into experimental and control groups, 40 cases in each. In control group, the regular infusion of glucose solution 500 - 1 000 ml for hydration be- fore chemotherapy, and rinsing the tube with 100ml saline after chemotherapy; in experimental group, on the basis of conventional care, hand dunking in warm water before chemotherapy, and after chemotherapy use Huolo oil external application on the upper limb associated with massage and exercise the upper limb with lifting upward and dilating chest. Observe and record the case of phlebitis and curing effect as well as venous puncture success rate of two groups within 5 courses of chemotherapy. Results The experimental group patients in the five courses of chemotherapy have less phlebitis cases and evidently priority in the respects of vein demonstration, elasticity, slight compression pain and smooth dripping than in the control group. Further more the venous puncture success rate of experimental group is higher than the control group with statistical significant difference p 〈 0. 01. Conclusion It shows that before chemo- therapy hands dunking in warm water, after chemotherapy Huolo oil external application along the vessel running posi- tion of upper limb associated massage and exercise of upper limb can effectively prevent chemotherapy phlebitis, elevate the success rate of venous puncture, and decrease the suffering of patients.
