Value of 3.0T Dynamic Contrast Enhanced MRI and Diffusion Weighted Imaging in Diagnosis of Prostatic Cancer

摘  要:目的:探讨3.0T MR动态增强扫描(DCE-MRI)及DWI技术对前列腺病变的鉴别诊断价值。方法:收集经穿刺活检、手术病理证实的60例前列腺疾病患者,其中前列腺癌34例,前列腺增生26例以及8例健康志愿者,均行MRI常规扫描、DWI及DCE-MRI扫描;选取感兴趣区记录动态增强定量参数K^(trans)、V_e和K_ep的值以及DWI的信号强度和表面扩散系数ADC值,并对得到的数据进行方差分析。结果:经DCE-MRI检查的34例前列腺癌患者中病灶区在DCE-MRI扫描的早期强化,定量参数K^(trans)、V_e和K_ep值分别为(0.337±0.029)/min,0.208±0.078,(0.701±0.052)/min;26例前列腺增生及8例正常前列腺的K^(trans)、V_e和K_ep值分别为:(0.263±0.041)/min,(0.171±0.034),(0.651±0.033)/min;(0.213±0.052)/min,(0.161±0.066),(0.634±0.054)/min,各参数值差异有统计学意义p<0.05;经DWI检查34例前列腺癌患者,病灶在DWI上呈高信号或稍高信号,ADC图上明显低信号,平均ADC值为(101.86±23.15)×10^(-5)mm^2/s,26例前列腺增生患者的平均ADC值为(165.34±56.23)×10^(-5)mm^2/s,前列腺癌、前列腺增生及正常腺体之间ADC值差异有统计学意义。结论:3.0T DCE-MRI及DWI技术在诊断前列腺病变中各自具有特征性表现,有助于对前列腺病变的鉴别诊断。Objective To explore the application value of dynamic contrast-enhanced MRI(DCE-MRI) and diffusion weighted imaging(DWI) in the diagnosis of prostate cancer(PC).Methods Thirty-four cases with PC and 26 cases with benign prostatic hyperplasia(BPH),confirmed by biopsy or operation,and 8 cases health volunteers underwent the examination of MRI conventional scanning,DCE-MRI and DWI.ROI were drawn to record the quantitative parameters values of K^(trans),V_e and K_(ep) The signal intensity of DWI and ADC value were measured respectively on the lesions of PC,BPH and normal prostatic zone. The results were statistically treated with ANOVA.Results The lesion showed obvious enhancement in the early phase of DCE-MRI in the 34 cases with PC,who underwent the examination of DCE-MRI.The K^(trans),V_e,K_(ep) value of cancerous foci was(0.337±0.029)/min,0.208±0.078,and(0.701±0.052)/min respectively,while(0.263±0.041)/min,0.171+0.034,(0.651±0.033)/ min for benign prostatic hyperplasia and(0.213±0.052)/min,0.161±0.066,(0.634±0.054)/min for normal prostatic zone.The differences were statistically significant(P0.05).The lesions showed lower signal intensity on ADC map in 34 cases with PC, who were examined with DWI and average ADC value was(101.86±23.15)×l0^(-5)mm^2/s.The average ADC value of the lesions of 26 cases with BPH was(165.34±56.23)×l0^(-5)mm^2/s.The statistical analysis showed that there were significant differences between prostatic cancer,benign prostatic hyperplasia and normal prostatic zone.Conclusion 3.0T DCE-MRI and DWI could present the specific finds and have a high value in the diagnosis of prostate cancer.
