摘  要:[目的]探讨孔径大小对多孔磷酸钙骨水泥(CPC)修复骨缺损能力的影响。[方法]用盐析法制备三种不同孔径(200~300μm、300~450μm、450~600μm)但孔隙率相同(68.3±3.3)%的多孔CPC材料,将材料植入兔大段桡骨缺损模型,术后4、12周进行大体观察、X线检查、血清骨源性碱性磷酸酶(BALP)检测、组织学观察和生物力学检测。[结果]术后4、12周X线检查可见200~300μm孔径材料骨缺损区愈合相对较好;4周时BALP检测200~300μm组高于其他组,差异具有统计学意义(P<0.05),12周BALP检测三组差异无统计学意义;12周组织形态计量学分析显示200~300μm材料组新生骨面积百分比最多,差异有统计学意义(P<0.05);力学结果显示200~300μm组材料的最大压缩载荷和弹性模量均好于其他两组,差异具有统计学意义(P<0.05)。[结论]孔径的大小可以影响骨生物材料在体内的成骨能力,本研究中较小孔径材料成骨能力更好,且更易达到成骨性能和力学性能的统一。[Objective]To investigate the effect of pore sizes of porous calcium phosphate cement on repair of large bone defect.[Method]Three kinds of CPC scaffolds with different pore sizes(200-300μm,300-450μm,450-600μm) but same porosity(68.3±3.3%)were manufactured by salt fractionation.We implanted the scaffolds into the rabbit large bone defect model.At 4 and 12 weeks after the operation,we took the general observation,X-ray examination,serum bone alkaline phosphatase(BALP) testing,histology and biomechanical testing.[Result]The X-ray examination showed that good bone healing occurred in the group which pore size was 200-300 μm in 4 and 12 weeks after the operation.The BALP values in 200-300 μm group were significantly higher than those in the other groups in 4 week(P0.05),but there were no significant difference among the three groups in 12 week.Histomorphology analysis indicated that the area of new bone formation in 200-300 μm group was larger than those in the others(P0.05).The mechanical test showed that the maximum compression load and elastic modulus in 200-300 μm group was significantly higher than those in the other groups(P0.05).[Conclusion]Pore sizes can affect osteogenesis of scaffolds in vivo.The smaller pore sizes of scaffolds in the present study can stimulate the new bone formation.
