臭氧髓核消融术联合高渗盐水盘内注射治疗腰椎间盘突出症

摘　　要：目的评价臭氧髓核消融术联合10%高渗盐水盘内注射治疗腰椎间盘突出症(LIDH)的疗效。方法选取356例LIDH患者,采用CT定位,以60μg/ml浓度的臭氧8～20ml注射后加注10%高渗盐水1.5～2.5ml于椎间盘内(臭氧联合高渗盐水组)。另选取279例LIDH患者进行单纯臭氧髓核消融术(单纯臭氧治疗组)。根据改良MacNab标准分级对比评价两组疗效。结果臭氧联合高渗盐水组显效213例(213/356,59.83%),改善126例(126/356,35.39%),无效17例(17/356,4.78%);其总有效率为95.22%(339/356),高于单纯臭氧治疗组(253/279,90.68%,P=0.024)。注射后无并发症发生。结论臭氧髓核消融术联合高渗盐水椎间盘内注射治疗LIDH有效、微创、安全。Objective To evaluate the therapeutic effect for the treatment of lumbar intervertebral disc herniation（LIDH） by intradiscal injection of O3 combined with 10% hypertonic saline.Methods Totally 356 patients with LIDH were selected,and 1.5—2.5 ml 10% hypertonic saline was injected into the lumbar intervertebral disc in addition with 8—20 ml 60 μg/ml O3（group A）.Group B included 279 patients with LIDH who received single intradiscal injection of O3.The therapeutic effect was assessed according to the reformative MacNab criteria and compared between the two groups.Results In group A,obvious effectiveness was obtained in 213 patients（213/356,59.83%）,effectiveness was observed in 126 patients（126/356,35.39%）,while ineffectiveness was found in 17 patients（17/356,4.78%）.The total effective rate of group A was 95.22%（339/356）,higher than that of group B（253/279,90.68%,P=0.024）.No complication occurred after the operation.Conclusion Intradiscal injection of O3 combined with hypertonic saline is an effective,minimally invasive and safe method for the treatment of LIDH.
