实时三维超声心动图评价原发性高血压患者右心室局部收缩功能

摘　　要：目的应用实时三维超声心动图(RT-3DE)技术评价原发性高血压(EH)患者右心室局部收缩功能。方法对80例右心室射血分数(EF)正常的EH患者(EH组)和40名健康志愿者(正常对照组)进行离线分析后,记录EH组和正常对照组右心室容积参数,并进行统计学分析。结果与正常对照组比较,EH组右心室整体、流出道部及流入道部EF减低(P均<0.05),而心尖肌小梁部EF差异无统计学意义(P>0.05);右心室整体舒张末容积、右心室整体收缩末容积减低,肌小梁部、流出道部、流入道部的舒张末容积及收缩末容积增高(P均<0.05)。与右心室整体EF相比,EH组内的肌小梁部、流出道部、流入道部EF均减低(P均<0.05),而心尖肌小梁部EF差异无统计学意义(P>0.05)。结论 RT-3DE可以准确评价EH患者右心室整体及局部容积与收缩功能。Objective To assess regional systolic function of right ventricular in essential hypertension （EH） patients with real-time three-dimensional echocardiography （RT-3DE）. Methods Eighty EH patients （EH group） with normal right ventricular ejection fraction （EF） and 40 normal controls （control group） were enrolled. Right ventricular volume parameters were off line assessed and statistical analyzed. Results The EF of global right ventricular, inflow and outflow in EH group were lower than those in control group （all P〈0.05）. However, EF of trabeculae apical myocardium was not significantly different between EH group and control group （P〉0.05）. Right ventricular end diastolic volume, right ventricular end systolie volume in EH group were lower than those in control group. Right ventricular regional end diastolic volume, right ventricular regional end systolic volume of trabeculae apical myocardium, inflow and outflow in EH group were higher than those in control group （all P%0.05）. EF of inflow and outflow were lower than that of global right ventricular （both P〈0.05） in EH group, whereas EF of trabeculae apical myocardium did not show evident difference （P〉0.05）. Conclusion RT-3DE can evaluate the global and regional right ventricular systolic function accurately in patients with essential hypertension.
