VCu与TCu380A宫内节育器用于有剖宫产史妇女的临床研究

摘　　要：目的比较VCu与TCu380A两种宫内节育器在有剖宫产史妇女中使用的效果和各自的特点，为育龄妇女知情选择避孕方法提供依据。方法2008年2月至2009年8月广东省阳江市计划生育服务中心对自愿要求放置宫内节育器（IUD）、身体健康且无上环禁忌证并有剖宫产史妇女400例，随机放置了VCu与TCu380A，其中放置VCu环200例，放置TCu380A200例，于放置后3、6、12个月进行随访。结果在12个月末VCu和TCu380A带器妊娠率分别为1．02％（2／196）和4．64％（9／194），脱落率分别为0（0／196）和3．1％（6／194），因症取出率分别为1．02％（2／196）和4．13％（8／196）。两种IUD妊娠率比较，差异有统计学意义（P〈0．05）；VCu脱落率和因症取出率均低于TCu380A，差异有统计学意义（P〈0．05）；VCu的累积续用率高于TCu380A（P〈0．05）。结论VCu避孕效果优于TCu380A，放置12个月时VCu未发生明显的不良反应，且具有放置方法灵活简便、脱落率低、因症取出率低、累积续用率高等优点，值得临床推广。Objective To study and compare the clinical efficacies and characteristics of VCu intrauterine device （IUD） and TCu380A IUD in childbearing-aged women with previous cesarean section so as to provide rationales for an informed choice of contraceptive methods. Methods According to their physical status, a total of 400 women undergoing previous cesarean section were randomly provided VCu IUD and TCu380A IUD from February 2008 to August 2009. These healthy women were had no contraindieation to IUD. The volunteers required voluntarily the placement of IUD. Then all of them were followed up for 3, 6 and 12 months. Results At Month 12, the pregnant rate with IUD in situ of VCu and TCu380A IUD were 1.02% and 4. 64%, expulsion rate 0 and 3.1% and withdrawing rate 1.02% and 4. 13% respectively. The pregnant rate was significantly different （ P 〈 0. 05 ） ; VCu showed a lower expulsion and withdrawing rate than that in TCu380A （ P 〈 0. 05 ） ; the continuation rate of VCu was higher than that of TCu380A （ P 〈 0. 05）. Conclusion VCu offers a better efficacy of contraception with a lower expulsion and withdrawing rate, a higher continuation rate and a simpler placement method.
