摘  要:目的探讨320排CT全肝灌注成像在临床应用的实用可行性及优越性。方法 10例正常肝脏、12例肝癌、7例肝转移瘤、9例肝血管瘤、2例血管瘤并发肝癌、1例转移瘤合并血管瘤均行全肝CT动态容积扫描,利用灌注软件获取兴趣区的灌注参数值,对正常肝脏各肝叶及肿瘤病变实性部分的灌注参数行统计学分析。结果正常肝脏各肝叶HAP、PVP及HAPI均无显著性差异,HAP与PVP比值在1/3~1/4间;肝癌、肝转移瘤、肝血管瘤病灶实性部分HAP及HAPI均有显著性差异,肝癌高于肝转移瘤及血管瘤,肝转移瘤位于两者之间,三者间PVP均有显著性差异,肝血管瘤高于肝癌及转移瘤。结论 320排CT动态容积全肝灌注成像可同时获取覆盖全肝的灌注数据,从整体观念来评价血供变化,对肝脏肿瘤及弥漫性病变的灌注分析有着明显的优势,具有明显的临床应用实用可行性。Objectives To explore the feasibility and superiority of whole-liver perfusion imaging by using 320 detector row CT.Methods Whole-liver dynamic volume scan were performed in 10 cases of normal liver,12 cases of hepatocarcinoma,7 cases of metastasis,9 cases of hemangioma,2 cases of hemangioma combine with hepatocarcinoma,and 1 case of hemangioma combine with metastasis.Perfusion parameter values of ROIs including each lobe of liver and the solid part of the each tumor were measured and statistical analyzed.Results No significant differences were found in HAP,PVP,and HAPI between each lobe of liver in normal liver;ratio of HAP to PAP was 1/3-1/4.There were significant differences in HAP,HAPI among the solid part of hepatocarcinoma,hemangioma and metastasis,hepatocarcinoma was the highest.There were significant differences in PVP,hemangioma was the highest.Conclusion Wholeliver dynamic volume CT scan by using 320 detector row CT can obtain the perfusion parameter value of whole organ at the same time,and evaluate the blood supply changes from the overall concept.It has obvious advantage in perfusion analysis of liver tumors and diffuse lesion and it's feasible in clinical use.
