前列腺特异抗原在前列腺疾病诊断中的应用价值探讨

摘　　要：目的探讨血清中总前列腺特异抗原(T-PSA)、游离前列腺特异抗原(F-PSA)与F-PSA/T-PSA比值在前列腺疾病诊断、疗效、预后中的应用。方法采用化学发光免疫分析法检测177例正常对照及205例良性前列腺疾病患者和72例前列腺癌(PCA)患者血清中的T-PSA、F-PSA,并计算F-PSA/T-PSA比值。结果 T-PSA、F-PSA在正常对照组与前列腺癌(PCA)组中比较,差异有高度统计学意义(P<0.01);在正常对照组与良性前列腺疾病组中比较,差异亦有统计学意义(P<0.05)。F-PSA/T-PSA比值在正常对照组与前列腺癌组中比较,差异有高度统计学意义(P<0.01);在正常对照组与良性前列腺疾病组中的比较,差异无统计学意义(P>0.05)。在转移复发组与治疗好转组中T-PSA、F-PSA,F-PSA/T-PSA比值比较,差异均有高度统计学意义(P<0.01)。结论 F-PSA/T-PSA比值结合T-PSA、F-PSA可提高对良性前列腺疾病与前列腺癌鉴别诊断的特异性,也可为临床医生观察临床疗效与预后提供可靠依据。Objective To investigate the application of serum total prostate-specific antigen （T-PSA）, free prostate specific antigen （F-PSA） and F-PSA/T-PSA ratio in the prostate disease diagnosis, prognosis, treatment application. Methods One hundred and seventy seven normal controls, 205 benign prostate patients and 72 prostate cancer patients （PCA） were employed, and T-PSA, F-PSA were detected in Chemiluminescence Immunoassay, and the F-PSA/T-PSA ratio was calculated. Results The difference of T-PSA and F-PSA between control group and prostate cancer （PCA） group was statistically significant （P 〈 0.01）, and in the control group with benign prostate disease group it was statistically significant （P 〈 0.05）. The F-PSA/T-PSA ratio in the normal control group and the prostate cancer group, the difference was statistically significant （P 〈 0.01）, the difference between the normal control group and the group of benign prostate disease-free was not statistically significant （P 〉 0.05）. In metastatic relapse group and treatment group in T-PSA, F-PSA, F-PSA/T-PSA ratio comparison, the differences were statistically significante （P 〈 0.01）. Conclusion The combination of F-PSA/T-PSA ratio with the T-PSA, the F-PSA can increase diagnosis specificity for benign prostate disease and prostate cancer, also for clinicians to observe the clinical efficacy and prognosis of providing reliable basis.
