摘  要:众多研究显示,丙泊酚对缺血再灌注损伤期间多脏器功能具有保护作用。它发挥保护作用的机制,主要通过丙泊酚对氧自由基、钙离子的调节、细胞因子、细胞凋亡等几方面的作用来实现,最近发现丙泊酚可减弱水通道蛋白的过度表达,从而减轻大脑局灶性缺血再灌注引起的脑水肿,也具有抑制线粒体通透性转换(MPT)的作用减轻心肌缺血再灌注损伤,还可明显下调脂多糖对肺泡巨噬细胞"高迁移率族蛋白B1(HMGB1)"启动子的转录激活,并使HMGB1 mRNA和蛋白表达降低,这为丙泊酚防治炎症疾病的机制研究提供了新的理论基础。A large number of researches have shown that propofol has protective effects on viscera function during ischemical reperfusion injury.Propofol plays its role through multiple mechanisms such as modulation of oxygen free radical and calcium,cell factors and cell apoptosis.Recently it has been found that propofol can weaken excessive expression of water channel thereby lessen the brain edema caused by brain focal ischemic reperfusion,and it can also inhibit the mitochondrial permeability transition(MPT) so reduce the myocardial ischemia reperfusion injury.Propofol can obviously down-regulate transcription activation of alveolar macrophage "high mobility groupbox-1 protein(HMGB1)" promoter induced by lipopolysaccharide and reduce the expression of HMGB1 mRNA and protein.These findings provide new theory basis for the prevention and treatment mechanisms of propofol for inflammatory diseases.
