摘 要:目的:考察延胡索合剂对吡柔比星外渗性损伤的防治作用及机制。方法:建立小鼠吡柔比星外渗性损伤动物模型;观察模型对照组、延胡索合剂1组(外渗后30 min湿敷)、延胡索合剂2组(外渗后d 3湿敷)和如意金黄散组(外渗后30 min湿敷)的外渗性损伤面积、程度和愈合时间;各组在外渗后d 1,4,7,11,18取1只小鼠,切除损伤组织,HE染色观察病理形态学改变,免疫组化法检测损伤部位皮肤及皮下组织中的血管内皮生长因子(VEGF)和表皮生长因子受体(EGFR)的表达情况。结果:延胡索合剂组和如意金黄散组的外渗性损伤面积及程度明显低于对照组(P<0.05)。与其余3组相比,延胡索合剂1组外渗性损伤面积明显缩小,愈合时间缩短(P<0.05)。镜下观察显示,延胡索合剂1组损伤程度轻,溃疡发生较晚,组织修复早,损伤组织中VEGF和EGFR的表达明显上调,高峰提前。结论:延胡索合剂对小鼠吡柔比星外渗性损伤有良好的防治作用,疗效优于如意金黄散,其促进损伤愈合的机制可能与促使早期损伤组织释放EGFR和VEGF,促进纤维组织增生有关。Objective:To investigate the effect and mechanism of Corydalis ambigua mixture on pirarubicin-induced extravasation injury.Methods:Kunming mice were used to establish the extravasation injury models induced by pirarubicin.Fourty extravasation injury mice were randomly divided into control group,Corydalis ambigua mixture group 1 and 2,Ruyi golden powder group.The area of extravasation lesion was measured and the degree and healing time of injury was recorded.The lesion tissues were resected from one mouse in each group on the d 1,4,7,11,18 after extravasation.Pathological manifestations were observed after HE staining.The expressions of vascular endothelial growth factor(VEGF) and epidermal growth factor receptor(EGFR) were detected by immunohistochemistry.Results:In the Corydalis ambigua mixture group 1,the area and the degree of extravasation injury were less than the other three groups(P0.05),and the healing time of injury was significantly shorter than the other three groups(P0.05).Pathological morphology showed a slighter tissue injury,delayed ulceration and earlier tissue repairment in Corydalis ambigua mixture group 1.Moreover,the expressions of VEGF and EGFR were higher in the Corydalis ambigua mixture group 1 than those in the control group.Conclusions:The preventive and therapeutic effects of Corydalis ambigua mixture on pirarubicin-induced extravasation injury are satisfactory,better than Ruyi golden powder.It may be associated to the up-regulation of VEGF and EGFR expression and fibrous tissue proliferation.
