胃大部分切除后胃瘫综合征28例临床治疗效果分析

摘　　要：目的探讨胃大部分切除手术后胃瘫综合征的临床治疗方法及效果。方法回顾性分析某院自2009年10月～2011年1月收治胃大部分切除术后出现胃瘫的患者28例,均给予禁食,持续胃肠减压,促胃肠动力药,营养支持治疗,3%温盐水洗胃,地塞米松以及中药大黄粉等。结果 28例患者均进行保守治疗,并于术后1～5周,恢复了胃肠系统动力,患者胃肠系统恢复的平均时间为12.4d。所在患者均进行随访,随访时间为3～7个月,平均时间为5.3月。随访期间没有出现复发。加用大黄粉会缩短康复时间。结论胃瘫术后的常见并发症,治疗多应采用以非手术综合治疗为主的措施,加用中药,可以有效地治疗腹部手术后的胃排空障碍,避免手术治疗,促进患者的全面修复。OBJECTIVE To evaluate the effect therapy of clinical effect of gastroplegia in postoperation on subtotal gastrectomy.METHODS We retrospectively analyzed 28 cases who had gastroplegia in postoperation on subtotal gastrectomy treated in our department from October 2009 to January 2011.All patients were given fasting and continuous gastrointestinal decompression,3% warm salt solution,gastrointestinal hyperkinesias drug,methylprednisolone and nutritional support treatment,etc.RESULTS All patients of 28 case were given expectant treatment.The symptom of functinoal delayed gastric emptying were improve in the 1-5 weeks,and the average time was 12.4 days.All cases were follow-up for three and seven months,and the average time was 5.3 month.CONCLUSION Gastroplegia were complication in the postoperation,non-operational composite treatment was main measure and could effectively treat gastroplegia in postoperation on subtotal gastrectomy.Expectant treatment can improve allover renovate,and avoid operation.
