摘 要:目的探讨血红素加氧酶-1(HO-1)对急性肺损伤(ALI)大鼠肺泡Ⅱ型上皮细胞(AEC-Ⅱ)的保护作用。方法雄性SD大鼠32只,随机均分为四组:正常对照组(C组),油酸损伤组(OA组),阿司匹林预处理组(AS组)和锌卟啉IX预处理组(IX组)。测定动脉血气,支气管肺泡灌洗液(BALF)和血清中肺表面活性物质相关蛋白A(SP-A)含量,肺组织湿/干重比(W/D);免疫组化法测肺组织HO-1蛋白表达,电镜下观察AEC-Ⅱ超微结构的变化。结果与C组比较,其余各组W/D、HO-1蛋白表达明显升高(P<0.01)。与OA组比较,AS组W/D明显下降(P<0.01),HO-1蛋白表达明显升高(P<0.01);IX组W/D明显升高(P<0.05),HO-1蛋白表达明显下降(P<0.01)。电镜显示OA组可见AEC-Ⅱ相对不规则,细胞变性甚至崩解,细胞表面微绒毛减少,胞浆内板层小体排空,部分排入肺泡腔。AS组较OA组轻;IX组较OA组重。结论 HO-1的高表达具有明显的保护AEC-Ⅱ的作用,进而对急性肺损伤有防治作用。Objective To study the protective effect of heme oxygenase-1(HO-1) on rat’s alveolar epithelial type Ⅱ cells in acute lung injury(ALI).Methods Thirty-two male SD rats weighing 180-200 g were randomly divided into 4 groups(n=8 each):group C(control group);group OA(injury group);group AS(aspirin pretreatment group);group IX(zinc porphyrin IX pretreatment group).The arterial blood gas,the SP-A content of bronchoalveolar lavage fluid(BALF) and serum,and the wet/dry(W/D)weight ratio of lung tissue were examined.The expression of HO-1 protein in the lung tissue and the ultrastructural changes of AEC-Ⅱ were also observed.Results There was significant difference in the wet/dry(W/D)weight ratio and the expression of HO-1 protein in all of the groups except group C(P〈0.01).Compared with group OA,the wet/dry(W/D)weight ratio decreased and the expression of HO-1 protein increased in group AS,but the wet/dry(W/D)weight ratio increased and the expression of HO-1 protein decreased in group IX.Relatively irregular AEC-Ⅱ,cell degeneration or disintegration,reduction of cell surface microvilli,intracytoplasmic lamellar body emptying,and part into the alveolar space were found by electron microscopy,with a severity sequence of group AS〈OA〈IX.Conclusion The high expression of HO-1 has an obviously protective effect on acute lung injury by acting on AEC-Ⅱcells.
