生物反馈电刺激对ⅢB型前列腺炎EPS中IL-10、PGE_2水平的影响及临床意义

摘　　要：目的:分析ⅢB型前列腺炎患者生物反馈电刺激治疗前、后前列腺液(EPS)中白细胞介素-10(IL-10)、前列腺素E2(PGE2)水平的变化以及与慢性前列腺炎症状的关系,探讨生物反馈电刺激治疗ⅢB型前列腺炎的作用机制。方法:50例ⅢB型前列腺炎患者作为观察组,非前列腺炎患者、尿常规及EPS常规检查正常者23例作为对照组,观察组采用生物反馈治疗仪治疗12周,检测治疗前、后国际前列腺炎症状评分(CPSI)、IL-10、PGE2水平,并与对照组进行比较。结果:治疗前观察组CPSI和EPS中PGE2水平均高于对照组,差异有统计学意义(P<0.05～0.01),EPS中IL-10水平差异无统计学意义(P>0.05)。观察组治疗后3个月和治疗结束后1个月CPSI和EPS中PGE2水平明显低于治疗前,差异有统计学意义(P<0.05),EPS中IL-10水平与治疗前相比变化不大(P>0.05),3个指标与对照组相比差异无统计学意义(P>0.05)。观察组EPS中的IL-10、PGE2与CPSI呈正相关(r值分别为0.563 5、0.432 9,P<0.01)。结论:ⅢB型前列腺炎患者EPS中IL-10和PGE2水平升高,其慢性盆痛症状严重程度与EPS中IL-10、PGE2水平呈正比。生物反馈电刺激治疗可能通过降低PGE2水平以改善ⅢB型前列腺炎患者的临床症状。Objective： To evaluate the role of IL-10 and PGE2 in expressed prostatic secretion（EPS） in the development of chronic prostatitis category Ⅲ B,and to investigate the effect of biofeedback pelvic muscles exercise in the treatment of Ⅲ B chronic prostatitis.Methods： A total of 50 patients with chronic prostatitis category Ⅲ B served as observation group,and 23 persons without prostatitis who had normal results in routine urine test and routine EPS test served as control group.Patients in observation group were treated by biofeedback therapy for 12 weeks,and chronic prostatitis syndrome index（CPSI）,IL-10 and PGE2 in EPS of observation group were observed before and after treatment,and were compared with control group.Results： Patients in observation group had significantly higher level of PGE2 and higher CPSI score than control group（P0.05-0.01）,but had no significant difference in level of IL-10 compared with control group（P0.05）.CPSI score and PGE2 were significantly lower 3 months after treatment and one month after treatment ends（P0.05）,but there was no significant difference in IL-10（P0.05）.Differences in those three indexes of observation group were insignificant compared with control group.There was a strong positive correlation between CPSI and IL-10 and PGE2（r=0.563 5 and 0.432 9,P0.05）.Conclusions：Patients with Ⅲ B prostatitis have elevated levels of PGE2 in EPS.The degree of pain is positively related with levels of IL-10 and PGE2.Biofeedback may improve symptoms via deceasing PGE2 level.
