摘 要:目的 探讨在磁共振3D T1 FFE序列中不同反转角度对三叉神经和周围血管成像质量的影响.方法 20例健康成年人为正常对照组,30例临床具有典型三叉神经痛症状的患者为实验组.2组双侧三叉神经脑干起始区域均行三维快速扰相梯度回波序列(3D T1 FFE)薄层扫描,反转角为:20°、25°、30°、35°、40°,扫描层厚/层间距=1.3 mm/-0.65 mm,测量正常组双侧三叉神经信号强度噪声比作为量化指标,由2位观察者采用双盲法对三叉神经显示影像质量进行评分,并测量实验组三叉神经长轴与邻近血管的距离.统计学采用单因素方差分析法.结果 (1) 反转角为20°时图像质量平均评分最高(2.70±0.10),信噪比最高(55.37),各组间比较均有显著性差异(P<0.01);(2) 反转角为20°时发现症状侧血管神经之间有切迹者8例,接触者14例,阳性率73.3%(22/30),非症状侧阳性率23.7%(7/30).有三叉神经痛症状的一侧血管接触、压迫神经的比率明显高于非症状侧(P<0.01).结论 反转角为20°时图像质量和信噪比最高,可清晰显示三叉神经与血管的关系.Objective To evaluate the effects of different flip angles at three-dimensional fast field echo (3D T1 FFE) sequence for examining trigeminal nerve and surrounding vascellum. Methods 30 patients witb neuralgia and 20 healthy persons were enrolled in our research. The bilateral trigeminal nerves in all cases underwent MR 3D T1 FFE sequence with different flip angles (20°, 25°, 30°, 35°and,10°) and slice thickness was 1.3 ram, slice gap was-0.65 ram. The relationship between trigeminal nerve and adjacent blood vessel was analyzed, and the distances between the trigeminal nerve and adiacent blood vessel were measured afterwards. The image quality in showing the trigeminal nerves was scored by two experienced radiologists in accordance with agreed guidelines. Results (l)When flip angle 20 ° was chosen, the average score in both the image quality and signal to noise(SNR) (2. 70±0. 10 anti 55.37, respectively) was the highest among different flip angles and there were statistically significant difference between them(P〈 0.01 ); (2)When using flip angle 20°, the indentations and contiguity between vessels and trigeminal nerves in disordered sides could be detected in 8 cases and 14 cases respectively,the positive rate was 73.3% (22/30), while the positive rate in the asymptomatic side was 23.3% (7/30),there was significance statistically between them( P〈0. 01). Conclusion The relationship between trigemi nal nerve and adjacent blood vessels can be detected sensitively with high image quality and SNR using the flip angle 20° at 3D T1 FFE.
