视功能问卷和生存质量问卷应用于早中期白内障手术适应证的研究

摘　　要：目的采用视功能问卷和生存质量问卷研究早中期白内障患者的视功能(visual function,VF)和生存质量(quality of life,QOL),探讨视力≥0.5早期白内障患者手术适应证。方法对日常生活视力≥0.3的161例早、中期白内障患者进行VF、QOL问卷调查,分为视力0.3、0.4组102例、视力≥0.5要求手术组21例、视力≥0.5未要求手术组38例,以视力0.3、0.4组VF、QOL得分95%位数为手术参考值,以另2组得分临床验证、筛选手术参考值。结果 161例受检者中,所有患者VF与日常生活视力显著相关(r=0.61,P<0.01),QOL与日常生活视力显著相关(r=0.45,P<0.01)。视力0.3、0.4白内障患者102例,95%位数VF得分为73.3分,QOL得分为94.4分。视力≥0.5患者59例,要求手术组21例,未要求手术组38例。≥0.5患者按VF参考值25例可手术,按QOL参考值43例可手术,与按患者实际要求手术手术率比较,VF为参考2种手术率无统计学意义(P>0.05),QOL为参考时2种手术率有统计学意义(P<0.05)。结论早中期白内障VF、QOL得分与日常生活视力显著相关,可将视功能问卷作为早期白内障患者的手术参考指征。Objective To study the visual function(VF) and quality of life(QOL) of the patients with early-middle stage of cataract by using VF and QOL questionnaires,and to analyze the operation indications of the early-middle stage of cataract [visual acuity(VA)≥0.5].Methods VF and QOL questionnaires were carried out in 161 patients with early-middle stage of cataract(presenting VA≥0.3).All the patients were divided into 3 groups: 1st group(VA 0.3-0.4) including 102 patients,2nd group(willing to receive operation and VA≥0.5) including 21 patients,and 3rd group(unwilling to receive operation and VA≥0.5) including 38 patients.The 95% percentiles of the VF and QOL scores of the 1st group were regarded as the operation indications of early-middle stage of cataract.The scores of the other two groups were used to verify and screen out the operation indications.Results The VF and QOL scores of all the 161 patients were significantly correlated with the presenting VA(r=0.61 and 0.45,P<0.01).The 95% percentiles of the VF and QOL scores were 73.3 and 94.4,respectively,in the 1st group.Operation could be given to 25 patients(VA≥0.5) according to the VF reference and to 43 patients(VA≥0.5) according to the QOL reference.Compared with the rate of actual operation requirement,the theoretical operation rate according to the QOL score had statistically significant difference(P<0.05),but that according to the VF score had not(P>0.05).Conclusion The scores of VF and QOL of the early-middle stage of cataract are significantly correlated with the presenting visual acuity.The VF questionnaire may be used as an operation indication for early-middle stage of cataract.
