摘 要:目的总结超声引导下肝癌瘤体内注射抗肿瘤免疫效应细胞的临床疗效。方法在GMP(Good Manufacturing Practice)条件下培养细胞因子诱导的杀伤细胞(CIK细胞、CD3AK细胞、γδT细胞和树突状细胞DC)。超声引导下在肝癌瘤体内多点注入抗肿瘤免疫效应细胞。治疗前后测定患者外周血T细胞亚群和绝对值,并随访观察瘤体大小及血供情况。结果 32例接受治疗患者共40个瘤体中,完全缓解(CR)+部分缓解(PR)+微效(MR)为22例(55%)。疗程结束患者CD3^+T细胞、CD4^+T细胞、CD4^+/CD8^+较治疗前明显升高,CD8^+T细胞较治疗有所下降。24例AFP升高的患者,治疗后15例降至正常,1例升高。食欲、体力改善者30例(93.75%),疼痛减轻者25例(78.13%),睡眠改善者16例(50%),体质量回升(大于治疗前体质量5%)11例(34.38%)。结论超声引导下肝癌瘤体内注射抗肿瘤免疫效应细胞,使肿瘤局部的免疫效应细胞浓度显著提高,可更加有效地杀灭肿瘤细胞。Objective To investigate the efficiency of ultrasonic-guided injection of effector T cells into hepatic carcinoma. Methods Killer cells (CIK cells, CD3AK cells, γδT cells and dendritic T cells) were cultivated and induced according to GMP standard. Effeetor T cells were injected into hepatocellular carcinoma guided by ultrasound. T-cell subgroup percentage and number were measured before and after the treatment. And the diameter and blood supply were followed up. Results In 32 patients ,which included fourty tumour lesions, 22(55%) were alleviated (including CR,PR and MR). The levels of CD3+T cells,CD4+ T cells, CD4+/CD8+ were significantly imcreased after the treatment, and CD8+ T cells were significantly decreased. In 24 patients with damaged liver function , AFP were recovered in 15 patients and increased in 1 patient. The appetite and physical strength were improved in 30 patients (93.75%). Pain and sleep were alleviated in 25 patients(78.13%) and 16(50%). Body weight was increased in 11 patients(34. 38%). Conclusions Local effector T cells were increased after the ultrasonic-guided injection of effector T cells into hepatocellular carcinoma. Thus carcinoma cells could be killed more efficiently.
