摘  要:目的:探讨早期肠内营养(EEN)联合重组人生长激素(rhGH)对慢性阻塞性肺疾病急性加重期(AECOPD)并发呼吸衰竭的疗效。方法:将90例AECOPD病人随机分为试验组和对照组,每组45例。对照组病人给予常规临床治疗,试验组病人在此基础上给予EN联合皮下注射GH,观察两组病人机械通气时间,检测两组病人治疗期间营养指标、免疫指标和T细胞亚群等。结果:试验组病人治疗后1周的血清清蛋白、转铁蛋白、前清蛋白、血红蛋白、纤维连接蛋白、免疫球蛋白和T细胞亚群均明显高于对照组(P<0.05),机械通气时间明显缩短(P<0.05),病死率减低(P<0.05)。结论:EN联合GH治疗不仅能缩短AECOPD并发呼吸衰竭病人机械通气时间,而且还能改善病人营养状况,增强免疫功能,提高临床疗效。Objective: To explore the effect of the early enteral nutrition and recombinant human growth hormone(rhGH) to the patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD) and respiratory failure.Methods: 90 patients with acute exacerbation of chronic obstructive pulmonary disease were randomly divided into experimental group(EN+rhGH)(n=45)and control group(n=45).The experimental group received the enteral nutrition and rhGH and the control group received the regular clinical therapy.The mechanical ventilation time was observed.The blood biochemical indexes,nutritional indexes immune indexes and T cell subset indexes in the period of treatment were examined in two groups.Results: In the experimental group,the albumins,transferrin,prealbumin,hemoglobin,fibronection,IgA and T cell subset were significantly higher than that of control group(P0.05)after a week,the mechanical ventilation time was shorter(P0.05),and mortality rate was lower(P0.05).Conclusion: Early enteral nutritional and rhGH can not only shorten the mechanical ventilation time,but also improve nutritional status and clinical effect in AECOPD patients.
