念珠菌与细菌混合感染引起足菌肿1例

摘　　要：报告1例由热带念珠菌、娄德罗念珠菌和金黄色葡萄球菌所致足菌肿。患者男,45岁。左足及左小腿红肿,足底破溃形成窦道伴溢液2个月。真菌直接镜检阴性。皮损组织病示:表皮下组织可见菌丝、孢子及颗粒物质。真菌培养有酵母样菌落生长,DNA测序鉴定为热带念珠菌和娄德罗念珠菌;细菌培养为金黄色葡萄球菌。诊断为足菌肿,给予伊曲康唑、阿奇霉素及局部外洗治疗后痊愈。A 45-year-old man had been suffered from flushing and swelling on the left shank and foot,forming sinus and purulence with thenar for two months.Fungal direct microscope examination appeared negative.Histopathology examination revealed hyphae,spores and granules.Fungal culture showed yeast-like fungus growing.The isolated strain was identified as Candida tropicalis and Lodderomyces elongisporus by ITS analysis.Bacterial culture displayed Staphylococcus anreus.The patient was dignosed as mycetoma.The lesions were improved by itraconazole and azithromycin systematically and other topical treatment.
