利福平改善糖皮质激素诱导的大鼠股骨头退变的组织学研究

摘  要:目的本研究探讨利福平(rifampicin)对糖皮质激素诱导的股骨头退变的作用。方法 40只SD大鼠随机等分为2组:A组以利福平每日10mg/kg灌胃5周,B组以生理盐水每日2mL灌胃5周,后4周两组均联用甲基强的松龙(methylprednisolone,MPSL)每日21mg/kg。第5周检测血清骨钙素含量、骨髓间充质干细胞(bonemarrow stromal cell,BMSC)的P糖蛋白(P-glycoprotein,P-gp)活性、股骨头内P-gp表达,并进行股骨头骨计量学和凋亡染色分析。结果 A组P-gp活性和表达量均高于B组(P<0.01)。与B组相比,A组骨化中心面积、骨小梁厚度及数量、破骨细胞和成骨细胞数量均有所增加(P<0.01);骨小梁分离度、脂肪细胞数量和周长、凋亡细胞数量减少(P<0.01)。结论利福平可减轻糖皮质激素诱导的大鼠股骨头退化样改变,该作用可能与其增强骨组织的P-gp活性和表达有关。Objective To investigate the effect of rifampicin on steroid-induced degeneration of femoral heads in rats.Methods Forty rats were equally divided into two groups:rifampicin(group A) and normal saline(group B) were administrated for 5 weeks;methylprednisolone(MPSL) was co-administrated subcutaneously in all the rats for the last 4 weeks.At the end of the 5th week,serum osteocalcin,P-glycoprotein(P-gp) activities of bone marrow stromal cells(BMSC) as well as P-gp expressions in the femoral heads were evaluated.We analyzed bone histomorphometry,adipocytic variables(adipose tissue area,adipocytic number and perimeter) and apoptosis in the femoral heads.Results Compared with group B,P-gp activity and expression significantly increased in group A(P0.01).Meanwhile,serum osteocalcin level,trabecular thickness and number,osteoclast and osteoblast number and mean percentage of epiphyseal ossification center also significantly increased in group A(P0.01).In contrast,adipocytic variables,trabecular separation,and apoptotic cells decreased significantlly in group A(P0.01).Conclusions Rifampicin inhibites the glucocorticoid-induced osteoporosis in rats,which may be associated with enhancing P-gp activity and expression in femoral heads.
