环磷腺苷葡胺联合丹红注射液治疗慢性收缩性心力衰竭的近期疗效评价

摘　　要：目的评价环磷腺苷葡胺联合丹红注射液治疗慢性收缩性心力衰竭的近期疗效。方法选择心功能Ⅲ～Ⅳ的慢性收缩性心力衰竭患者82例，将其随机分为治疗组和对照组。两组患者均给予常规抗心力衰竭治疗（病因治疗，应用洋地黄、利尿剂、血管紧张素转换酶抑制剂、血管扩张剂等）。治疗组加用环磷腺苷葡胺120mg＋5％葡萄糖溶液250ml静脉滴注，1次／d；联用丹红注射液20ml＋生理盐水250ml静脉滴注，1次／d，疗程14d。治疗前、后测定血流动力学指标、实验室指标，观察临床症状及体征变化，评价疗效。结果治疗组总有效率为87．5％，对照组总有效率为71．4％，两组疗效差异具有显著性（P〈0．05）。治疗14d后，治疗组的心率、6MWT及各项血流动力学指标比对照组改善明显，差异有显著性（P〈0．05）。两组患者治疗前、后肝功能、肾功能、血常规、电解质及心电图检查未见明显异常变化。结论在常规抗心力衰竭治疗的基础上，加用环磷腺苷葡胺联合丹红注射液可在短期内明显改善患者的心率、6MWT及各项血流动力学指标，无明显不良反应。Objective To evaluate the short-term efficacy of meglumine cyclic adenosine combining with Danhonginjection for chronic systolic heart failure. Methods 82 patients with chronic systolic heart failure and Ⅲ～Ⅳ ventricular function were randomly divided into study group and control group. Both groups received conventional therapies for heart failure （ etiological treatment and uses of digitalis, diuretics, and angiotensin converting enzyme inhibitors, and vasodilators ） the study group was additionally administered intravenous meglumine cyclic adenosine of 120mg plus 5% glucose solution of 250 ml combining with Danhong injection of 20 ml plus normal saline of 250 ml daily, 14 days for each course. Hemodynamic parameters and laboratory indexes were determined and the changes in clinical symptoms and the sign were observed to assess the efficacy. Results The total effectiveness rate differed signficantly between the study group and the control group （ 87.5% vs. 71.4%, P〈 0.05 ）. 14 days after treatment, the changes in heart rate, 6MWT, and all the hemodynamic parameters were more obvious in the study group than in the control group, with a significant difference （ P〈 0.05 ）. No markedly abnormal changes in liver function, kidney function, routine blood test, electrolytes, and ECG examination were found after treatment. Conclusions Apart from regular therapies for heart failure, meglumine cyclic adenosine combining with Danhong injection can evidently improve heart rate, 6MWT, and all the hemodynamic parameters in a short term, and has no apparent adverse reactions.
