摘  要:目的:探讨糖化血红蛋白(HbA1c)及糖化血清蛋白(GSP)在妊娠糖尿病早期诊断的临床应用价值。方法:选取384例孕妇为研究对象,按临床诊断分为妊娠糖尿病组(GDM组)144例、糖耐量受损组(IGT组)87例及正常妊娠对照组153例,比较三组别糖筛查、HbA1c、GSP的血清水平及GDM组、IGT组中HbA1c、GSP的异常率,分析HbA1c、GSP在GDM诊断的方法学指标评价。结果:三组别糖筛查HbA1c、GSP血清水平差异均有统计学意义(P<0.05),组间两两比较差异均有统计学意义(P<0.05);IGT组、GDM组的组间、组内HbA1c、GSP的异常率差异均有统计学意义(P<0.05),GSP有较高的灵敏度,HbA1c有较高的特异度及阳性预测值。结论:糖化血红蛋白及糖化血清蛋白在妊娠糖尿病早期诊断有重要的临床应用价值。Objective To investigate the glycosylated hemoglobin(HbAlc) and glycosylated serum protein(GSP) in the early diagnosis of gestational diabetes mellitus clinical application value.Methods 384 Cases of pregnant women as the research object,according to the clinical diagnosis of gestational diabetes group(GDM group),144 patients with impaired glucose tolerance group(IGT group) and 87 cases of normal pregnancy 153 cases in the control group,compared with three groups of glucose screening,HbA1 c,GSP levels in serum and GDM group,IGT group in HbAlc,GSP HbAlc,abnormal rate,analysis of GSP in the diagnosis of GDM methodology evaluation.Results Three groups of glucose screening results,HbAlc,serum GSP level through analysis of variance,there was significant difference between the three groups,two two had statistical differences(P 〈 0.05);GSP HbAlc,the abnormal rate of two groups and each group by chi-square test,there was significant difference(P 〈 0.05),GSP has high sensitivity and HbA lc has high specificity and positive predictive value.Conclusion Glycosylated haemoglobin and glyeosylated serum protein in the early diagnosis of gestational diabetes mellitus has important clinical application value.
