睾丸扭转14例报道并文献复习

摘　　要：目的提高睾丸扭转的临床诊治水平。方法回顾性分析14例睾丸扭转患者的临床资料,所有患者均以阴囊疼痛就诊,术前均行阴囊彩超检查,均表现为患侧睾丸肿大,睾丸血流消失12例,血流减少2例。结果 10例(71.4%)患者行睾丸切除,其中7例仅行患侧睾丸切除(50%),另3例行患侧睾丸切除并对侧睾丸固定;其余4例行双侧睾丸固定。结论 Doppler超声检查可准确检测出睾丸血供,是一种快速、简便、无创伤和可重复的临床检测方法。早期手术探查可提供准确的诊断和治疗,降低睾丸切除的风险。Objective To improve the clinical levels of diagnosis and treatment of testicular torsion.Methods The clinical data of 14 cases of testicular torsions were summarized and analyzed.All the patients had the essential clinical symptoms of scrotal pain.Color Doppler sonography showed no or less blood perfusion in all cases compared to contralateral testis,of these patients,12 with no perfusion,and 2 with decreased perfusion.Results 10 cases（71.4%）of necrosis testis underwent orchiectomy,of these patients,7（50%）had unilateral orchiectomy only and 3 simultaneously had contralateral testis orchiopexy;and the other 4 patients had bilateral testis orchiopexy.Conclusion It is helpful to get exact and early diagnosis of testicular torsion by color Doppler sonography,which is a fast,convenient and no injury repetitive clinical method.Earey surgical exploration can offer an accurate diagnosis and treatment,minimizing the risk of testicular orchiectomy.
