摘 要:目的探讨农村社区原发性高血压患者血清γ-谷氨酰转肽酶(gamma-glutamyltransferase,GGT)与代谢综合征(metabolic syndrome,MS)组分相关性。方法纳入连云港市农村社区45~75岁原发性高血压患者12 879人,进行病史询问、血压测量、体检,并采集静脉血以检测血糖、血脂等生化指标。结果总人群GGT水平中位数为23.0(17.0,34.1)U/L,其中男性29.0(21.0,46.0)U/L;女性20.0(15.0,28.9)U/L。GGT水平升高(≥40 U/L)占总人群18.8%,其中男性占32.6%;女性占10.7%。MS患病率为41.5%(男性:24.0%,女性51.7%)。总人群中GGT水平升高的OR(95%CI)在甘油三酯(triglyceride,TG)水平升高组、空腹血糖(fasting plasma glucose,FPG)升高组及MS患病组分别为2.37(2.16~2.61)、2.01(1.83~2.20)和1.42(1.30~1.56)。多元线性回归显示,GGT水平与FPG(男性:β=0.314 2,P<0.001;女性:β=0.434 9,P<0.001),TG(男性:β=0.369 1,P<0.001;女性:β=0.248 0,P<0.001)密切相关。结论原发性高血压患者血清GGT水平与TG、FPG水平及MS相关。Objective To explore the relationship between serum γ-glutamyltransferase(GGT) and metabolic syndrome(MS) components in patients with primary hypertension in Lianyungang rural communities.Methods 12 879 patients with primary hypertension,aged 45-75 years and living in Lianyungang rural communities,were recruited in this research.Data were collected from the questionnaire,blood pressure measurement and physical examination.Serum samples were collected to detect blood glucose and lipid.Results Median of GGT level in total population was 23.0(17.0,34.1) U/L [men:29.0(21.0,46.0) U/L;women:20.0(15.0,28.9) U/L],GGT levels(≥40 U/L) accounted for 18.8%(men:32.6%;women:10.7%).The prevalence of MS in the total population was 41.5%(men:24.0%,women:51.7%).The OR(95%CI) for elevated GGT in the total population were 2.37(2.16-2.61),2.01(1.83-2.20)and 1.42(1.30-1.56)in the patients with high triglyceride(TG) concentration,fasting plasma glucose(FPG) and MS respectively.Multiple linear regression analysis showed that GGT levels was closely related to FPG(man: β = 0.314 2,P〈0.001;woman: β = 0.434 9,P〈0.001),and TG(man: β = 0.369 1,P〈0.001;woman: β = 0.248 0,P〈0.001).Conclusions GGT is positively correlated with TG,FPG and MS in patients with primary hypertension.
