性别对PCI术后患者服药依从性和生活质量影响的研究

摘　　要：目的比较不同性别PCI术后患者服药依从性和生活质量的差异。方法采用问卷调查法对403例PCI术后患者(其中男性242例,女性161例)的服药依从性和生活质量进行评定,Morisky问卷调查PCI术后服药依从性、西雅图心绞痛量表和SF-36评定PCI术后半年生活质量。结果不同性别患者对Aspirin、BBs、ACEI/ARB这三类药物依从性差异具有统计学意义(P<0.05),女性较男性依从性好,但对Statins类药物依从性差异无统计学意义(P=0.051)。不同性别患者SF-36总体得分差异无统计学意义(P>0.05),但在躯体功能、生理职能、躯体疼痛及躯体健康方面差异有统计学意义(P<0.05),女性得分较男性高;SAQ得分女性在躯体活动受限程度、心绞痛发作情况、心绞痛稳定状态方面得分较男性高(P均<0.05),而男性对疾病的主观感受高于女性(P<0.05)。结论对PCI术后患者应针对不同性别采取相应的健康干预措施,男性主要在改善其遵医行为方面,女性侧重于精神心理方面。Objective To compare medication adherence and quality of life between male and female patients after percutaneous coronary intervention.Methods Medication adherence was measured by Morisky questionnaire and quality of life was measured by Seattle angina questionnaire and SF-36.Results Medication adherence of Aspirin,BBs and ACEI/ARB was significantly different between different genders（P0.05）and women were better than men while the adherence of Statins was of no significantly difference（P=0.051）.The total score of SF-36 had no significantly gender difference but among physical function,role-physical,bodily pain and physical health system the scores of female patients were significantly higher than men（P0.05）.So as to SAQ scores,women were significantly higher in the dimensions of physical limitation,anginal frequency,and anginal stability than men.But men had better disease perception than women and showed significantly difference（P0.05）.Conclusion Specific health measures should be adopted to suit different gender.Male patients should focus on improving abiding medical behaviors and female on mental health intervention.
