摘 要:目的探讨高渗盐复苏液对严重烫伤大鼠早期血钠及细胞因子的影响。方法 56只SD大鼠随机分成3组:对照组、乳酸钠林格液组(LR组)和高渗钠盐液组(HS组)。所有大鼠全部颈静脉置管,对LR组和HS组大鼠进行烫伤后复苏治疗,分别检测各组肺脏含水量、血钠、肿瘤坏死因子-α(TNF-α)、白介素6(IL-6)水平,将所得数据进行统计学分析。结果 HS组复苏后各时间点的肺含水量明显低于LR组(P<0.05或P<0.01),两组明显低于对照组(P<0.05或P<0.01);HS组复苏后各时间点的血钠明显高于LR组(P<0.01),HS组与对照组比较,差异无统计学意义,LR组明显低于对照组(P<0.05或P<0.01);HS组复苏后各时间点TNF-α及IL-6与LR组的TNF-α及IL-6比较,差异均无统计学意义,两组均明显高于对照组的TNF-α及IL-6(P<0.05或P<0.01)。结论与乳酸钠林格液比较,早期使用高渗盐液进行液体复苏严重烫伤大鼠能明显减轻肺水肿与低钠血症。Objective To explore the effect of sodium hypertonic fluid on blood sodium and cytokines of severely scalded rats in the early stage. Methods Fifty-six SD rats were randomly divided into three groups: control group, sodium lactate Ringer's solution group (LR group) and hypertonic sodium solution group (HS group). Neck vein catheterization was used on all rats, and scald resuscitation therapy was adopted in the rats of LR group and HS group. Moisture content of lungs, blood sodium, tumor necrosis factor-α ( TNF-α ) and interleukin 6 ( IL-6 ) were examined in three groups. The data drawn from the test were statistically analyzed. Results The moisture content of lungs decreased significantly in the HS group versus the LR group ( P 〈 0.05 or P 〈 0. 01 ), and increased significantly in the group HS and blood sodium increased significantly in the group LR compared with the control group (P 〈0. 05 or P 〈0. 01 ). The HS group versus the LR group ( P 〈 0. 0 1 ) , and decreased significantly in the LR group compared with the control group (P 〈 0.05 or P 〈 0. 01 ). There was no significant difference in the blood sodium between the control group and the HS group. The levels of TNF-α and IL-6 increased significantly in the HS and LR group versus the control group ( P 〈 0. 05 or P 〈 0. 01 ). There was also no significant difference in the levels of TNF-α and IL-6 between the HS group and the LR group. Conclusion Compared with LR, supplementing HS can ameliorate significantly pneumonedema and hyponatremia of severely scalded rats in the early stage.
