超声引导下肋间神经阻滞治疗胸背部带状疱疹

摘　　要：目的 观察超声引导下肋间神经阻滞治疗胸背部带状疱疹的疗效。方法 胸背部带状疱疹患者60例，随机分为肋间神经阻滞组（A组）和阿昔洛韦组（B组）。每组30例。两组均用阿昔洛韦5mg／kg加入5％GS500ml静脉滴注3次／d；A组再在超声引导下肋间神经阻滞5～7d1次。于术后各时点观察VAS评分以及镇痛、止疱、结痂时间与痊愈率。结果两组间VAS评分、止痛、止疱、结痂时间及痊愈率均有统计学差异（P〈O.05）。结论胸背部带状疱疹采用阿昔洛韦治疗的同时给予超声引导下肋间神经阻滞镇痛效果明显；可缩短止痛、止疱、结痂时间，提高治愈率。Objective To observe the effects of intercostal nerve block guided by ultrasound on patients with herpes zoster in chest and back. Methods Sixty patients with herpes zoster in chest and back were randomly divided into intercostal nerve block group （group A） and acyclovir group （group B）, 30 cases in each group. Acyclovir 5 mg/kg and 5 %GS 500 ml were intravenously infused every 8 hours in both groups, meanwhile the patients were given ultrasound-guided intercostal nerve block once every 5-7 days in group A. VAS, pain relieving time, herpes disappearing time, scab forming time and the cure rate were observed after the treatment. Results There were significant differences in VAS, pain relieving time, herpes disappearing time, scab forming time and the cure rate between the two groups （P〈0.05）. Conclusion Ultrasound-guided intercostal nerve block combined with acyclovir improves the pain relieving time, herpes disappearing time, scab forming time and increases the cure rate in patients with herpes zoster in chest and back.
