摘  要:目的:观察不同电针频率下针药复合麻醉对肺切除术患者细胞因子表达的影响,为针刺麻醉电针频率的选择提供依据。方法:将择期行开胸肺切除术56例患者随机分为Ⅰ组(22例)、Ⅱ组(16例)、Ⅲ组(18例)。各组均行常规药物麻醉,全麻前30min,Ⅱ组、Ⅲ组予电针后溪、支沟、内关、合谷,Ⅱ组予连续波型,频率2Hz,Ⅲ组予疏密波型,频率2Hz/100Hz,电针刺激维持至手术结束。采用双抗体夹心免疫酶标法(ELISA)检测患者术前24h、切皮后10min、术后24h血清白介素1(IL-1)、白介素6(IL-6)含量。结果:3组患者切皮后10min IL-6较术前24h显著上升(均P<0.01),IL-1含量变化不大(均P>0.05);与切皮后10min比较,Ⅰ组与Ⅲ组患者术后24hIL-1含量显著上升(均P<0.01),Ⅱ组则变化不大(P>0.05);3组患者术后24hIL-6含量较切皮后10min均显著上升(均P<0.01)。结论:2Hz电针频率针药复合麻醉能部分调节体内炎性因子的表达,从而减轻围术期机体的应激反应,保护脏器功能。Objective To observe the influence of acupuncture-drug compound anesthesia with different frequency electroacupuncture on cytokine expression in patients with pneumonectomy in order to provide evidence for selection of electroacupuncture frequency in acupuncture-drug compound anesthesia.Methods Fifty-six patients scheduled for pneumonectomy were randomly divided into groups Ⅰ(n=22),Ⅱ(n=16) and Ⅲ(n=18).Gene-ral anesthesia was produced in all the groups.The patients in group Ⅱ and Ⅲ were treated with electroacupuncture at Houxi(SI 3),Zhigou(TE 6),Neiguan(PC 6) and Hegu(LI 4),30 min before general anesthesia.The 2 Hz continuous wave was produced in group Ⅱ,and the 2 Hz/100 Hz was produced in group Ⅲ till the operation finished.The serum content of interleukin 1(IL-1) and 6(IL-6) was detected by double antibody sandwich enzyme linked immunosorbent assay(ELISA) at the time of 24 hours before operation,10 minutes after incision and 24 hours after operation.Results The contents of IL-6 in all the three groups at the time of 10 minutes after incision were all higher than those before operation(all P0.01),and the content of IL-1 had no obvious change(all P0.05).In comparison with 10 minutes after incision,the contents of IL-1 in group Ⅰ and Ⅲ were both significantly increased at the time of 24 hours after operation(both P0.01),and there was no obvious change in group Ⅱ(P0.05).The contents of IL-6 in all the three groups at the time of 24 hours after operation were more significantly increased than those at the time of 10 minutes after incision(all P0.01).Conclusion Acupuncture-drug compound anesthesia with 2 Hz frequency can partly regulate the expression of inflammatory factors so as to alleviate the stress reaction in perioperative patients and protect organs function.
