补中益气汤加味治疗妇女压力性尿失禁临床研究

摘　　要：目的探讨补中益气汤加味治疗妇女压力性尿失禁(SUI)的临床疗效。方法选择香港地区成年女性SUI患者58例,随机等分为A组和B组,采用两阶段交叉设计,A组先行盆底肌运动疗法后服用中药补中益气汤加味,B组先服用中药后行运动疗法,治疗后比较疗效。结果第一阶段治疗前后情况采用两独立样本t检验,运动组泌尿生殖简表(UDI)、视觉模拟量表(VAS)评分、咳嗽测试、盆底肌力量治疗前后差异有统计学意义(P<0.05),失禁频率、终止测试、盆底肌耐力及协调能力差异无统计学意义(P>0.05);中药组各指标治疗前后差异均有统计学意义(P<0.05)。疗效评价采用两阶段交叉设计资料的方差分析,两种治疗方法间UDI、VAS评分、失禁频率、咳嗽测试的差异均有统计学意义(P<0.05),终止测试、盆底肌力量、盆底肌耐力及协调能力差异无统计学意义(P>0.05)。结论补中益气汤加味与盆底肌运动疗法都能有效控制SUI症状,前者较后者可能疗效更佳,但尚不能认为在改善患者身体机能方面优于后者。Objective To evaluate the clinical effect of Buzhong Yiqi Decoction in the treatment of stress urinary incontinence（SUI）.Methods A total of 58 SUI patients were divided randomly into group A and group B equally,adopted the two stage cross-over design.Group A had the kinesiotherapy first and took traditional Chinese medicine later,on the contrary,group B took traditional Chinese medicine first and had the kinesiotherapy after that.The efficacy of the two groups was observed after the treatment.Results After the treatment of first stage,we adopted two independent sample T-test to compare the two groups separately.In kinesiotherapy group,UDI,VAS,cough test and pelvic floor strength before and after treatment were significantly different（P0.05）,but there was no difference in incontinence frequency,termination test,pelvic floor endurance and coordination ability（P0.05）.In Chinese medicine group,all targets were significantly different（P0.05）.The curative effect appraisal used two stage cross-over design variance analysis,the difference in UDI,VAS,incontinence frequency,cough test between two treatment methods were significant（P0.01）,but there was no difference on termination test,pelvic floor strength,pelvic floor endurance and coordination ability（P0.05）.Conclusion Both of Buzhong Yiqi Decoction and kinesiotherapy are the efficacious methods in treating SUI.Buzhong Yiqi Decoction may has better effect than kinesiotherapy,but there is still no enough evidence to prove that Buzhong Yiqi Decoction is better than kinesiotherapy in improving physical function of patients.
