摘 要:目的评估男男性行为人群(MSM)HIV-1感染者外周血中自然杀伤(NK)细胞亚群和受体的变化,并初步探讨NK细胞与HIV-1疾病进展的关系。方法选取45例未经抗病毒治疗的MSM HIV-1早期感染者和35名健康对照人群。分离外周血单个核细胞(PBMC),并用多色流式细胞检测技术分析NK细胞亚群和受体表达的变化。结果与正常对照相比,HIV-1感染者NK细胞杀伤亚群显著减少(P=0.002),抑制性受体CD161表达明显降低(P<0.001),但功能不明亚群和活化性受体NKG2C和NKP46明显增加(P<0.001)。与CD4计数和病毒载量的相关性分析显示,仅NKG2A的表达与CD4计数呈正相关,其余无明显相关性。以上NK细胞的亚群和受体表达的变化特征可发生在HIV急性感染期(<3个月)。结论 HIV急性感染可影响NK细胞亚群重分布及表型特征,提示NK在HIV急性感染阶段即可发挥重要作用。Objective To characterize the subsets and cell surface receptors of natural killer(NK) cells in the early stage of human immunodeficiency virus type 1(HIV-1) infection in the population of men haveing sex with men(MSM) and the relationship with the disease progression.MethodsPeripheral blood mononuclear cells(PBMC) from 45 treatment-nave MSM patients of early HIV-1 infection and 35 healthy donors were isolated.The subsets and surface receptors of the NK cells were analyzed using multi-color flow cytometry.ResultsThe cytotoxic NK cell subset and the expression of inhibitory receptor CD161 in the HIV-1 infection group are significantly lower than that in the normal control group(P=0.002 and P0.001,respectively).The dysfunctional NK cell subset and the expression of activating receptors(NKG2C and NKP46) in the HIV-1 infection MSM group were significantly higher than that in the control group(P0.001).A positive correlation was found between NKG2A with CD4 counts in the analysis of NK cell receptors,CD4 counts,and viral load.ConclusionsSignificant changes in NK cell subsets and expression of cell surface receptors can occur at the early stage(within 3 months of infection) of acute HIV-1 infection,indicating that NK cells play critical roles in the development of HIV-1 infection and disease progression.
