摘 要:目的研究探讨基质金属蛋白酶-1(MMP-1)基因1G/2G多态性在支气管结核人群中的分布及其与支气管结核狭窄的关系。方法采用DNA测序方法,对114例支气管结核患者和110例单纯肺结核患者进行MMP-1基因1G/2G多态性的基因型分析,比较基因型分布和支气管结核组织浸润和治疗后狭窄指标的关系。结果 MMP-1基因1G/2G多态的1G/1G、1G/2G和2G/2G基因型在支气管结核患者中的分布频率分别为44.7%、38.6%、16.7%;1G/2G多态性基因型分布与支气管结核治疗后支气管狭窄显著相关(P=0.003);与2G/2G纯合子基因型相比,携带1G等位基因的支气管结核患者治疗后发生支气管狭窄情况更严重(P<0.05);1G/2G基因型分布与支气管结核患者的年龄、性别、支气管结核分型无明显相关。结论 MMP-1基因1G/2G多态性与治疗后支气管结核的狭窄性有关,携带1G等位基因可能是支气管结核治疗后支气管狭窄的危险因素。OBJECTIVE To investigate the association between 1G/2G polymorphism in MMP-1 gene and its developing tracheobronchial stenosis of endobronchial tuberculosis.METHODS A total of 114 endobronchial tuberculosis patients and 110 pulmonary tuberculosis patients were included in the study.Genotype analysis was performed by using DNA sequencing method.Bronchoscopic examination was performed to determine the presence of endobronchial involvement.The relationship between this SNP(single nucleotide polymorphism) and M.tuberculosis invasinveness was evaluated by comparing bronchoscopic features of the patients with endobrochial tuberculosis.RESULTS The distributions for 1G/1G,1G/2G and 2G/2G genotype of MMP-1 1G/2G polymorphism were 44.7%,38.6%,and 16.7%,respectively.A significant correlation was found between the genotype distribution and developing tracheobronchial stenosis of endobronchial tuberculosis(P=0.003);the patients with 1G/1G or 1G/2G genotype carriers had greater risk of developing tracheobronchial stenosis than the ones with 2G homozygous genotype carriers(P〈0.05).1G/2G polymorphism was not found any significant difference among the groups with the age,gender and clinical type on the basis of its bronchoscopic features.CONCLUSION 1G/2G polymorphism in MMP-1 gene promoter may play a role in aggressiveness of endobrochial tuberculosis and 1G allele could be associated with increased risk of developing tracheobronchial stenosis of endobronchial tuberculosis through up-regulation of MMP-1 activity.
