补肾健脾利湿中药在体外受精—胚胎移植周期中防治卵巢过度刺激综合征的临床研究

摘　　要：【目的】研究补肾健脾利湿中药在体外受精—胚胎移植(IVF-ET)周期中防治卵巢过度刺激综合征(OHSS)的效果。【方法】筛查IVF-ET超促排卵周期OHSS高危因素患者68例,随机分为中药组(37例)和对照组(31例),2组应用相同的超促排卵方案,中药组加用补肾健脾利湿中药。比较2组的临床各项指标及OHSS的发生情况。【结果】2组患者的年龄、体质量指数(BMI)、促性腺激素(Gn)用量、Gn天数、获卵数、应用人绒毛膜促性腺激素日(HCG日)雌二醇(E2)水平及临床妊娠率等比较差异均无统计学意义(P>0.05)。胚胎移植日中药组OHSS发生率(21.6%)显著低于对照组(87.1%),差异有统计学意义(P<0.05);中药组新鲜周期胚胎移植率(94.6%)显著高于对照组(45.2%),差异有统计学意义(P<0.05)。2组共19例患者为了预防中、重度OHSS的发生进行了全部胚胎冻存。【结论】对于IVF-ET周期OHSS高危患者加用补肾健脾利湿中药,能显著降低OHSS发生率和OHSS严重程度,增加OHSS高危患者新鲜周期胚胎移植率。Objective To observe the preventive and therapeutic effect of Chinese herbal medicine on ovarian hyperstimulation syndrome（OHSS） in in-vitro fertilization and embryo transfer（IVF-ET） cycle.Methods The total 68 patients with high risk of OHSS in IVF-ET cycle were randomly divided into Chinese herbal medicine group（N=37） and control group（N=31）.The two groups received the same hyperstimulation regimen,and Chinese herbal medicine group was given the herbs with the actions of tonifying kidney,strengthening spleen and eliminating dampness additionally.General clinical data and the incidence of OHSS were compared in the two groups.Results There were no significant differences among the two groups in the age,body mass index（BMI）,the total dosage of Gn ampouls,the medication days of Gn,the number of oocytes,serum estrogen level on the day of using human chorionic gonadotrophin（HCG）,and clinical pregnancy rate（P0.05）.The incidence of OHSS in Chinese herbal medicine group was 21.6%,which was significantly lower than 87.1% in the control group（P0.05）.Thirty-five patients in Chinese herbal medicine group and 14 in the control group went into fresh IVF-ET cycle,and there was significant difference of the rate of embryo transfer in fresh IVF-ET cycles between the two groups（94.6% vs 45.2%,P0.05）.Embryo freezing was performed in 19 patients for the prevention of moderate or severe OHSS.Conclusion The application of kidney-tonifying,spleen-strengthening and damp-eliminating Chinese herbal medicine can significantly decrease the incidence and severe degree of OHSS,and increase embryo transfer rate in fresh IVF-ET cycles in patients with high risk of OHSS in IVF-ET cycle.
