单侧输尿管结扎大鼠肾组织胰岛素样生长因子-1表达及其与肾间质纤维化的相关性研究

摘　　要：目的研究单侧输尿管结扎(UUO)模型中胰岛素样生长因子-1(IGF-1)和基质金属蛋白酶-2(MMP-2)的表达趋势及其与肾间质纤维化的关系,以及用胰岛素增敏剂曲格列酮的干预效果。方法采用UUO模型,将90只SD雄性大鼠,随机平均分为3组,即:假手术组(行手术分离但不结扎左侧输尿管)、模型组(手术结扎左侧输尿管近肾盂段)和曲格列酮治疗组。分别在术后第1、3、7、14天观察肾小管受损的程度,用免疫组织化学法检测IGF-1和MMP-2在肾间质纤维化中的表达。结果 模型组IGF-1及肾小管间质损伤程度明显高于假手术组,随着梗阻时间的延长,IGF-1表达逐渐上调,而MMP-2的表达逐渐下降;治疗组与模型组相比IGF-1表达下降,而MMP-2表达上调。相关分析显示,IGF-1与MMP-2呈负相关(r=-0.890,P<0.01)。结论 IGF-1和MMP-2的表达趋势的变化与肾间质纤维化的发生、发展有重要的意义,而曲格列酮有明显的干预效应。Objective To investigate the expression trends of insulin-like growth factor -1 （IGF-1） and matrix metalloproteinase-2 （MMP-2） and their correlations with renal interstitial fibrosis in rat models with unilateral ureteral obstruction （UUO）, and to evaluate the intervention effects of insulin-sensitizing agent-troglitazone. Methods Ninty SD male rat models with UUO were established and randomized into 3 groups, including sham operation group （without ligation in left ureter）, model group （with ligation in left ureter near the section of renal pelvis） and troglitazone treatment group. All rat models were observed their degrees of renal tubule injury at 1, 3, 7, 14 days after surgery. Immunohistochemistry was used to detect the expressions of IGF-1 and MMP-2 in renal interstitial fibrosis. Results In model group, IGF-1 expression and the degree of tubulointerstitial injury were significantly higher than those in sham operation group. The expression of IGF-1 was increased along with time of obstruction, while the expression of MMP-2 was gradually decreased. On contrary, the troglitazone treatment group had a decrease in the expression of IGF-1 but an increase in the expression of MMP-2 as compared with model group. The relative analysis demonstrated that IGF-1 expression was negatively correlated with MMP-2 expression （r=-0.890, P0.01）. Conclusion The expressive trends of IGF-1 and MMP-2 may be of great significance in the occurrence and development of renal interstitial fibrosis, and troglitazone was found to have significant intervention effects.
