腹腔镜手术治疗青春期女性巨大卵巢囊肿20例

摘　　要：目的探讨腹腔镜治疗巨大卵巢囊肿的可行性、有效性和安全性。方法根据临床表现、术前超声、肿瘤标记物等检查,初步判断卵巢囊肿的性质,术中先行穿刺缩瘤,为腹腔镜手术创造条件,再行腹腔镜下卵巢囊肿剥除术或附件切除术。结果 20例均在腹腔镜下完成,手术时间平均62 min,术中出血量平均56 ml,住院时间(5±2)d。无术中、术后并发症,术后随访最长8年,无一例复发。结论腹腔镜治疗巨大卵巢囊肿具有创伤小、恢复快等优点,安全可行。Objective To study the feasibility,efficacy and safety of laparoscopic treatment for giant ovarian cyst.Methods According to preoperative ultrasonography,tumor markers and clinical manifestations,initial risk assessment of benign and malignant ovarian cyst was made.The ovarian cyst was punctured to shrink tumors,thus creating conditions for laparoscopic surgery.Then,the ovarian cyst and/or accessories were removed in laparoscopic resection.Results 20 cases were completed in laparoscopic;The operating time was 62 min,the volume of intraoperative bleeding was 56 ml,and the length of stay in the hospital was（5±2） days.There were no complications in operation and postoperation.No recurrence was reported during the follow-up for up to 8 years.Conclusion For giant ovarian cysts,laparoscopic treatment has advantages of small trauma,fast recovery,safety and feasibility.
